For the subjective testing of refraction, Edward J ackson1 believes that the inter national broken ring test is inferior to the test letters; but for testing acuteness of vision, it is distinctly bet ter. When, however, the broken ring is arranged like test letters in successive lines of varying sizes, it has no advantage over the test letters, and some disadvantages. It is when a single size of broken ring is arranged in a symmetrical card that its advantages of greater accu racy and convenience appear. So arranged it is to be used by varying the distance of the test from the person to be tested. A set of such cards is presented, of four sizes. On one the broken ring is 72. 5 millimeters in diameter. To see the break in the ring at five meters indicates vision of The other card has rings of 36. 2 and the other two cards indicate vision of 1. At 5 meters and vision of 2. At 5 meters. A table is pre sented giving the indications of these four tests at vari ous distances. To use this test, the card is held close to the patient, the break in the ring pointed out, and he is asked to indicate its direction. When he understands fully what is expected, the card is taken to a distance, the break in the ring turned in some other direction and the patient asked to indicate its new position. If unable to see the ring at that distance, it is brought nearer until the direction of the break is recognized. Excerpt from A System of Practical Medicine, Vol. 2 of 3: Comprised in a Series of Original Dissertations A sensation of dryness in the mucous membrane of the eyes and nostrils is a not unfrequent attendant upon amaurosis. Pain limited to the eyes, or extending to the head and face, is observed in the majority of amaurotic cases. It may be 'constant, intermittent, or periodic. Its character, degree, and the extent over which it' is felt, ought to be inquired into; also, the circumstances which tend to relieve or aggravate it, whether accompanied with vertigo, tinnitus aurium, nausea, a disposition to sleep, or wakefulness. Dul ness of one or more of the senses sometimes exists, with failure of memory, and inability to exert certain other of the intellectual faculties. The history of the patient's general health, and especially of any previous attacks of disease, ought to be made a particular subject of inquiry, as tending to unfold the causes which may have remotely and indirectly prepared the way for his present affection. Other points for investigation arising out of the peculiarities of each individual case, will readily suggest themselves to every reflecting mind. H. F. Hansell,1 after examining Mariotte's spot in thirty seven eyes of emmetropes, eleven hyperopes and four myopes, finds that the distance from the center of the blind spot to the fixation point is a little longer in the right than in the left eye of emmetropes; in hyperopia the average distance is 3 mm. Longer in the left than in the right eye, while the longest and shortest distance is almost the same in each eye. In myopia the cases were so few that the aver ages may be misleading. In comparing the three varieties of refraction we find that the greatest as well as the shortest distance is found in myopia. The center of the blind spot is, with a few exceptions, below a horizontal line running through the point of fixation. The shape of the blind spot in emmetropia is oval, with the long diameter in the ver tical axis. In four of the cases the outlines were circular and in two the long axis was horizontal. This would seem to show that the optic disc is seldom circular, but, in the majority of cases, oval. In emmetropia the long diameter of the blind spot averages 31 mm.; in hyperopia, 34 mm. And in myopia, 38 mm. It will thus be seen that the size of the blind spot is distinctly greater in myopia than in any other state of refraction. Hansell believes that an ex amination of the size of the blind spot will prove useful in cases of optic neuritis, choked disc and glaucoma as well as in other disturbances of the eye. Regeneration of the Ciliary Nerves. A. Biette2 found that a few years after an opticociliary neurectomy had been performed, the eye had again become sensitive. An an atomic examination having been made, he discovered that a regeneration of the ciliary nerves had occurred. New formations can spring from the nerve trunk, he says, and in spite of defective coaptation may penetrate the sclera and lead to irritative symptoms, especially in the ciliary body. Naturally the editor of the part devoted to Pharmacology and Therapeutics shares the opinion of his distinguished colleagues. Like the others he has been embarrassed by the enormous volume of material poured in upon him when the cessation of the war reopened the floodgates of European literature. It required the exercise of much care to select some and reject the rest of it. As a general principle, work of no practical therapeutic value has not been reviewed. Demonstrations of the worthlessness of certain new and little used remedies, important as these studies are to the pioneer in therapy, have generally been rejected owing to lack of space. Wherever the editor entertained grave doubts about the soundness of an author's conclusions, the article has been withheld instead of indulging in derogatory editorial comments, as might have been his privilege.
