Our usual social Ego is oriented horizontally. In the vertical, there has only been spirit or God from above and drive and affect from below. Just recent psychoanalytical studies have described the very early body-self-mirrorings, according to which the infant still remains largely in volved in itself. Even in adult life these experiences of morroring the inner vertical, or better espressed as a vertical Ego, still play an important role. The author shows this with many examples, but also describes a self-therapeutic procedure that is built up from seemingly such contradictory elements as psychoanalysis and meditation. According to the instructions, anyone can learn it themselves by means of two exercises.

Social Structures is a book that examines how structural forms spontaneously arise from social relationships. Offering major insights into the building blocks of social life, it identifies which locally emergent structures have the capacity to grow into larger ones and shows how structural tendencies associated with smaller structures shape and constrain patterns of larger structures. The book then investigates the role such structures have played in the emergence of the modern nation-state. Bringing together the latest findings in sociology, anthropology, political science, and history, John Levi Martin traces how sets of interpersonal relationships become ordered in different ways to form structures. He looks at a range of social structures, from smaller ones like families and street gangs to larger ones such as communes and, ultimately, nation-states. He finds that the relationships best suited to forming larger structures are those that thrive in conditions of inequality; that are incomplete and as sparse as possible, and thereby avoid the problem of completion in which interacting members are required to establish too many relationships; and that abhor transitivity rather than assuming it. Social Structures argues that these "patronage" relationships, which often serve as means of loose coordination in the absence of strong states, are nevertheless the scaffolding of the social structures most distinctive to the modern state, namely the command army and the political party.

In the decade-and-a-half since I coedited Transsexualism and Sex Reassignment (Green & Money, 1969), remarkable changes have occurred with Harry Ben jamin's "transsexual phenomenon" (1966). Formerly, when writing about this condition in scientific journals, it was necessary to define the term transsex ualism. Now the lay public recognizes it. Even the American Psychiatric Asso ciation acknowledges it as a "disorder," with its inclusion in the Third Edition of its Diagnostic and Statistical Manual (1980). Although this "elevation" to the status of mental illness may seem a Pyrrhic victory, it is a recognition of the legitimacy of transsexual ism as a source of human suffering. The controversy that surrounded the decisions in the early patient cases to perform sex-change surgery has largely dissipated. The cries of "collusion with delusion," principally from psychoanalysts, have quieted. The dire predictions of psychosis and/or suicide following surgery as the "last psychic defenses are cut away" have almost never been realized. By contrast, many postoperative patients consider the surgery to have been life-saving. Medical centers worldwide have incorporated programs for evaluating and treating persons requesting sex reas signment. Elaborate guidelines for patient management have been developed by an international organization of health care professionals (Harry Benjamin Inter national Gender Dysphoria Association, 1981). Harry Benjamin's child has come of age.
