The common and unfortunate complication after cardiac arrest is hypoxic-brain injury, which leads to devastating neurological complication. There are only 7% to 30% of the patient, discharged at home with good neurological outcome, who were admitted after cardiac arrest. The initial emergency treatment of stabilizing the heart and the intensive care unit (ICU) management to treat the underline cause of the cardiac arrest are still not resulting in improved neurological outcome or decrease mortality rate in post cardiac arrest patient. The induction of therapeutic hypothermia (TH) of 32 ?C ' 34 ?C in ICU within 3-4 hours for 24 hour and then rewarm slowly at temperature of 0.3- 0.5 ?C every hour, with injunction of the other care management and treatment for post cardiac arrest patient. Evidences has suggested that therapeutic hypothermia can provide cerebral neuro-protective benefits in post cardiac patients even with different methods of hypothermia induction. The treatment process to desired outcome requires close monitoring of the patient, competent nurses with knowledge and skills of the treatment. The therapeutic hypothermia is concurrently implemented in hospitals for the short term and long term patient quality patient care outcome. Since 2007, the year hypothermia was included in treatment plan for post cardiac arrest by different hospitals, the mortality rate of these hospital has significantly decreased and the neurologic disability also decreased among the survivors overtime.

Therapeutic hypothermia has emerged as a very important treatment option for patients with cardiac arrest as it provides significant protection from developing neurologic injury once the patient has been successfully resuscitated. Studies have demonstrated over 15% absolute risk reduction in death and neurologic injury using this therapy. Although hospitals and medical centers have become familiar with this important intervention it still remains greatly under utilized due to an experience and lack of resources to safely and effectively deploy such a program. The objective of this book is to educate and familiarize both providers and institutions as to how to develop and deploy and provide therapeutic hypothermia to their patients. The current knowledge for this is provided by speakers and national experts and also by literature review. There are several courses being provided on this as well throughout the US. These are good venues for people to come and see and get hands on experience, but there still needs to be a concrete book with references on how to go about getting this program started.

Purpose: Therapeutic hypothermia (TH) reduces the damage by ischemic / reperfusion syndrome in cases of cardiac arrest, in which its application is already widely established and carried out in centers of excellence. However its use in patients with ST segment elevation myocardial infarction (STEMI) is still scarce. The simulation decreases the gap between what is taught in theory and its application in professional practice. The aim of the trial was the development of a simulation protocol and training of the multidisciplinary team to perform therapeutic hypothermia as an adjunctive treatment in patients with STEMI, and validation of the method by applying in real cases.Materials and Methods: Use of modern mannequins with realistic simulation situations and different scenarios for the treatment of patients with STEMIs undergoing therapeutic hypothermia as an adjunctive treatment, from the emergency room, through the cath lab, and to the intensive care unit (ICU). There were 36 multidisciplinary professionals doing the training among realistic simulation with modern mannequins and real scenarios in sectors of the hospital where the real patients would be treated.Results: The focus of simulation and training was the logistics optimization and debriefing with strategies to reduce waste of time in the patient's transportation between the various departments, and avoiding excessive reheating during transport between sectors. There was absolute success in the realization of therapeutic hypothermia and validation of the method in real life, with the application of knowledge and logistics in 10 real patient without any delay in the doo-to-balloon time for primary angioplasty, which occurred in a timely manner ( less than 90 minutes), and maintenance of therapeutic hypothermia in the ICU successfully.Conclusions: Simulation was an important tool related to the training and optimization of health professionals skills, and improving the multidisciplinary team to perform therapeutic hypothermia in STEMIs. The use of real scenarios and debriefing were critical to the successful implementation of TH in practice. After simulation and training, the protocol and the method were validated with application of TH in real life successfully and without any delays.

Cardiac arrests are responsible for about 325,000 deaths annually in the United States. The majority of these cardiac arrests occur outside the hospital setting, with a dismal national survival rate of 8%. The Bernard (2002) and HACA (2002) studies demonstrated that by initiating therapeutic hypothermia as a treatment modality for cardiac arrests the survival rate could be as high as 49% and 55% respectively. Therapeutic hypothermia can be initiated in many different settings. The question remains, does the setting of the initiation of therapeutic hypothermia effect patient outcomes? The purpose of this study was to compare the effects of initiation of therapeutic hypothermia by EMS, Emergency Departments, and ICU nurses on cardiac arrest patient outcomes as measured by the Glasgow Coma Scale at discharge. This study utilized a convenience sample of 178 consecutive cardiac arrest patients admitted from 2008-2010 in an 800-bed hospital in Western North Carolina. From the overall sample, 57 patients had a favorable neurological outcome (32% overall survival rate). EMS initiated therapeutic hypothermia in 24 instances with 7 patients surviving to discharge (29% survival rate). The Emergency Department initiated therapeutic hypothermia in 17 instances with 8 patients surviving to discharge (47% survival rate). ICU nurses initiated therapeutic hypothermia in 137 instances with 42 patients surviving to discharge (31% survival rate). Study results reproduced previous findings demonstrating the efficacy of therapeutic hypothermia in the treatment of cardiac arrests. The results also indicate a possible advantage to the initiation of therapeutic hypothermia by Emergency Departments.

Therapeutic hypothermia is a lesser used, promising intervention for the preservation and/or improvement of neurological functions following cardiac arrest and return of spontaneous circulation. This research study summarizes the results of multiple studies, which show an up and coming correlation between the use of therapeutic hypothermia and increased survival and good neurological outcomes of patients who experienced cardiac arrest in or out of the hospital setting. The use of therapeutic hypothermia at Desert Valley Hospital was analyzed and an education plan to increase and improve the frequency and use of therapeutic hypothermia was instilled. The chief of medical staff, along with the directors of the emergency department (where the therapy is started) and the intensive care unit (where the therapy is maintained and discontinued) and the educator of the hospital were collaborated with regarding the implementation plan. The research article reviews were used to develop the education materials to teach and familiarize the staff with the therapy. Knowledge of therapeutic hypothermia research findings and how to use the therapy were low within the facility, and contributed to the lack of its use. Lack of knowledge of the benefits of therapeutic hypothermia as well as lack of awareness of recommended practices, perception of poor prognosis after cardiac arrest, increased effort required for therapeutic hypothermia, and staffing shortages, result in the decreased use of the therapy. (Lang and Arrich, 2010) Pre and post policy implementation patient outcomes, (survival rates and neurological outcomes) and staff competency were compared by using data collection, quizzes and surveys.
