With more than 850 clinical photographs in a text/atlas format, Tissue Expansion in Reconstructive and Aesthetic Surgery describes over a hundred cases of tissue expansion in reconstructive surgery. Newer applications of tissue expansion in aesthetic surgery are discussed. The areas of expansion include the breast, scalp and forehead, face, nose, ear, extremities, abdomen, back, scrotal area, and other specialized areas of expansion. Intraoperative expansion and complications are also covered. * Describes more than 100 clinical cases, and includes preoperative, intraoperative, and postoperative photographs so readers can see the progression of each case. * Includes more than 850 color photographs so that readers can view the procedures step by step, including primary and secondary operations that are necessary to complete the reconstruction. * Discusses both chronic expansion and intraoperative expansion. * Thoroughly covers breast reconstruction, with discussions of indications for tissue expansion and more than 180 photographs. * Includes several complications chapters that discuss reaction patterns and dysfunctional changes. Additionally, each clinical chapter discusses possible complications in detail. * Covers the use of endoscopy as an alternative to conventional expansion techniques.

Tissue expansion is a relatively straightforward procedure that enables the body to "grow" extra skin for use in reconstructing almost any part of the body. A silicone balloon expander is inserted under the skin near the area to be repaired and then gradually filled with salt water over time, causing the skin to stretch and grow. Almost anyone in need of additional skin can benefit from tissue expansion, from infants to elderly.

Reconstruction of oral and maxillofacial defects is challenging. Insufficient soft tissues may render hard tissue reconstruction problematic. Several surgical techniques have been used over time to address this issue; these techniques are usually complicated and unpredictable. Soft tissue expansion is a physiological process that leads to the formation of new cells and growth of tissue and allows for soft tissue with similar colour, texture and function to that of the adjacent tissues. In this article we present the applications of osmotic tissue expanders in facilitating bone graft augmentation. OSMED (Ilmenau, Germany) self-inflating tissue expanders were used prior to bone augmentation in our patients. After making a 1.5 cm full thickness incision, a sub-periosteal tunnel was prepared and the tissue expander was implanted sub-periosteally. The tissue expanders were removed approximately 6-10 weeks later in the course of augmentation surgery. In all patients after the use of the tissue expander, sufficient soft tissue was available for primary, tension-free, wound closure and there was no need for local or regional flap techniques. No complications such as infection, necrosis, or graft loss occurred and the functional and esthetic outcomes were acceptable. Use of this tissue expander prior to bone augmentation was effective in facilitating bone graft augmentation.

This thesis aims to investigate a possible approach to increase the volume of the area in which augmentation material can be inserted under oral soft tissues with self-inflating osmotic soft tissue expanders. This then allows more augmentation material to be inserted. The properties of the augmentation material inserted, will have impact to the bone genesis. Therefore, bone morphogenetic protein 2 (BMP-2), which is known to be osteo-conductive and osteo-inductive, is studied. It is unclear whether there is bone formation when using BMP-2 in the treatment of peri-implantitis. The most effective dose BMP-2 to promote bone formation when using an osseo-conductive coralline hydroxyapatite (CHA) coated with an osseo inductive layer biomimetic calcium phosphate (BioCaP) is not clear yet. In order to describe the ideal properties of an expander for intra oral use, literature on tissue expanders used in general medicine was studied. Tissue expansion is indicated in cases where other surgical techniques aimed at gaining the soft tissue surface are inadequate. The use of tissue expanders requires extra time, extra surgical procedures, money and discomfort for the patient which limits its clinical use. The used self-inflating hydrogel Osmed ® expanders were enveloped in a silicone hull with a few small perforations, through which tissue fluids can be adsorbed by the hydrogel. Fixation of the expander, which is possible with a screw through the extension of the silicone envelope prevents dislocation. The histological findings showed no difference at comparing expanded and unexpanded tissue. The increase of the amount of soft tissue was clinically clearly visible, and no differences were found when following either the flap placement or the tunnel placement procedure. In a case report the use of two Osmed ® hydrogel soft tissue expanders enveloped in a silicone hull is described.
