In this volume, prestigious geriatric researchers examine the concepts and practice of Total Quality Management and Continuous Quality Management as innovative approaches to improving health care services and delivery to the elderly. The authors supply case illustrations for the implementation of quality improvement processes in care settings for older adults, such as nursing homes, home care, acute care, and office practice, as well as in the training of medical students and postgraduates. Demonstrations are given as to how different approaches can be effectively applied to each setting.

The emergence of "total quality" as an approach to organising, sustaining and improving quality of service offers considerable potential benefits to the health service in the 1990s and beyond. TQM is an integral part of effective management, not an optional extra. It is a system which in general results in greater awareness of how performance in delivering health care can be explicitly measured against agreed standards to the satisfaction of both patients and "purchasers". TQM is a comprehensive approach, both cultural and technical, and must embrace all staff, all disciplines and all activities. This text offers a description of what TQM is, how the quality culture can be developed, what the contributory technical components are and the benefits which can accrue from implementing this important organisational initiative.

Total Quality Management (TQM) is a set of concepts, tools and applications which has been so successful in manufacturing industry that we are now witnessing experimentation in the transference of Total Quality Management to the public sector provision of government, health and education in North America, Europe and elsewhere. TQM is starting to set a new paradigm for management approaches in the public sector and "not for profit" enterprises. All key public service managers should at least need to know the basics of TQM, its possibilities and limitations for the public sector, and particularly the types of applications which could work for them. For all public sector managers this book provides: a clear understanding of the key concepts of TQM; a critical understanding of their relevance to the public sector; empirical evidence of TQM applications in government, health and education; and exploration of the public sector TQM possibilitites yet to be realized. It draws throughout on case examples from Britain, Canada, the USA and continental Europe which illustrate the application of TQM to the public sector.

As the Baby Boomers age, concerns over healthcare systems' abilities to accommodate geriatric patients grow increasingly challenging. This is especially true with the population deemed to be “the oldest of the old,” specifically those over the age of 85. Unlike any other time in history, this demographic is the fastest growing segment of most developed countries. In the United States the oldest old is projected to double from 4.3 million to 9.6 million by 2030. The increased life expectancy of the population since the early 1900s has been built on the improvement of living conditions, diet, public health and advancement in medical care. With this we have seen a steady decline in the age-specific prevalence of vascular and heart diseases, stroke and even dementia. Older persons are healthier today than their counterparts decades ago. More importantly than in any other age group, the care of the oldest old must be individualized; management decisions should be made taking into consideration the older persons’ expressed wishes, quality of life, function and mental capacity. The inevitable consequence is that there will be an increase in the prevalence of older persons with chronic diseases, multiple co-existing pathologies and neuro-degenerative diseases. The oldest of the aging population are often excluded from drug trials and their treatments are largely based on findings extrapolated from that of the younger old. Furthermore, among the oldest old, physiologically they are more diverse than other segments of the population. Their demographic characteristics are unparalleled and different compared to that of the younger old. Several studies have drawn attention to the differing attitudes among health professionals towards elderly people and many show prejudice because they are old. As a result, the use of age as a criteria in determining the appropriateness of treatment is of very limited validity, yet there are limited resources that guide physicians through these challenges. This book creates a greater awareness of these challenges and offers practical guidelines for working within the infrastructures vital to this demographic. This book is designed for geriatricians, primary care physicians, junior medical officers, specialty geriatrics nurses, and gerontologists. It is divided into 3 sections: General Considerations, Chronic diseases and Geriatric Syndromes. Each chapter provides a summary of important and essential information under the heading of Key Points. Case studies are included in some of the chapters to highlight the principles of management.

This book offers an up-to-date review on the principles and practice of multidimensional assessment and management of the older individual, which represents the cornerstone of modern clinical practice in the elderly. The early chapters cover the main elements and scope of the comprehensive geriatric approach and explain the pathways of care from screening and case finding through to in-depth assessment and treatment planning. Subsequent chapters review the evidence of how best to apply the multidimensional assessment and management approach in defined healthcare settings and within specific clinical areas, such as cancer and surgery. Finally, the education and training challenges are reviewed and the prospects for future clinical service and research in this important field are examined. The book is very timely given the recent advances in application of this approach, which reflect the growing international realization that older people are “core business” in many clinical areas where the role of specialist geriatric medicine has hitherto been limited. Accordingly, the book will be relevant to a wide range of clinicians. The authorship comprises many of the best known and widely published experts in their respective fields.
