Pengobatan luka ulkus diabetikum membutuhkan beberapa jenis terapi, yaitu terapi untuk menurunkan kadar glukosa darah, terapi untuk luka seperti dressing dan debridement jaringan nekrotik secara berulang, serta membutuhkan terapi antibiotik untuk infeksi dari ulkus diabetik tersebut. Salah satu obat penurun kadar glukosa darah adalah metformin. Metformin adalah obat golongan biguanida yang merupakan lini pertama untuk pengobatan diabetes melitus tipe 2, dengan profil keamanan yang sangat baik, kemanjuran yang tinggi dalam kontrol glikemik dan manfaat kardioprotektif yang jelas namun belum sepenuhnya dipahami. Obat ini banyak digunakan karena keamanannya dan biayanya yang murah. Telah digunakan selama lebih dari 60 tahun untuk mengobati diabetes tipe 2 pada tahap awal karena kemampuannya yang luar biasa untuk menurunkan kadar glukosa plasma. Indonesia memiliki 30.000-50.000 spesies tanaman yang 9.606 diantaranya digunakan sebagai tanaman obat dan dewasa ini penelitian mengenai tumbuhan obat tersebut telah banyak dikembangkan sesuai khasiat kandungannya. Banyak jenis tanaman yang dapat dibudidayakan karena bermanfaat bagi manusia dalam hal pengobatan. Komposisi dalam ekstrak tanaman banyak terdapat komponen kimia yang dapat digunakan sebagai obat. Salah satunya adalah Suruhan atau Peperomia pellucida L. Kunth. Suruhan (Peperomia pellucida L. Kunth) merupakan salah satu tanaman obat yang umum digunakan di Indonesia. Suruhan bisa ditemukan di pekarangan, ladang, dinding, dan area lain yang cukup lembab,. Tinggi tanaman ini sekitar 6 sampai 45 cm, dan bentuk daun menjadi melebar di pangkal dan menyempit di tepi daun. Tanaman yang termasuk dalam famili Piperaceae ini memiliki banyak sekali manfaat dan khasiat untuk kesehatan tubuh, diantaranya digunakan secara empiris untuk mengobati demam, sakit kepala dan sakit perut. Tumbuhan ini dapat digunakan sebagai anti bakteri, anti inflamasi, dan analgesik. Beberapa hasil penelitian yang telah dilakukan menunjukkan bahwa tanaman suruhan mempunyai potensi sebagai antiinflamasi, antipiretik, antimikroba dan anti kanker, dan memiliki efek analgetik
