The Thoracic Surgery Residents Association (TSRA) was established in 1997 under the guidance of the Thoracic Surgery Directors Association (TSDA) to create a unified voice to represent residents during cardiothoracic surgery training. The organization has developed as a core mission a focus on improving the quality, accessibility and utility of resident education. The current publication seeks to provide a resource which may be used in the surgeon's daily practice as a resource for the review of the major steps of an operation as well as a guide to the appropriate documentation of the conduct of that operation. We hope that the text will be useful for both practicing surgeons and residents in training. While the list of operations included in the text is extensive it is by no means intended to be inclusive of the entire breadth of the field of cardiothoracic surgery. Additionally, the text provides descriptions of one approach to the operations included. We acknowledge that the approach to a given operation may vary among surgeons, and institutions. This text is not intended to establish a uniform 'correct' approach, nor is it intended as a surrogate for formal residency training.

Over the last 23 years our identity and mission of the TSRA have expanded to meet the needs of our members by providing peer-based resources and support for cardiothoracic surgery residents to succeed throughout training and launch successful careers. The prior edition of this book, TSRA Clinical Scenarios in Cardiothoracic Surgery, published in 2013 was tremendously successful. The first edition of this book aimed to augment available resident educational resources. It focused on knowledge application in a dynamic decision-making capacity highlighting surgical technique, practical knowledge and clinical acumen based on a variety of different clinical case scenarios. This edition aims to update this review resource while simultaneously expanding the scope of clinical scenarios to keep pace with the ever-changing clinical cardiothoracic training environment.The aim of this book is to review common, high-yield clinical scenarios that may surface during a cardiothoracic surgeon's practice in congenital, thoracic, or adult cardiac surgery. It was written by residents for residents. We meticulously reviewed and edited all previous clinical case scenario chapters focusing on updated clinical and scientific evidence. The first edition contained 72 chapters authored by over 50 residents and faculty from across the country. The new edition contains 19 new chapters (36-chapter revisions) with 91 chapters authored by 159 residents and faculty nationwide! We have maintained the structure and flow of the book utilizing the three major sections of cardiothoracic surgery: General Thoracic Surgery, Adult Cardiac while expanding the congenital section to be Congenital Cardiothoracic Surgery.

Operative Dictations in General and Vascular Surgery, Second Edition is intended to teach residents the principles of succinct and precise operative dictations for a wide spectrum of surgical procedures. Designed as a portable resource, the book provides typical dictations to guide the resident. Comprised of 226 procedures, this new edition comprises the majority of procedures commonly performed by general and vascular surgeons. For each procedure, a list of common indications is given. This list covers 95% of the situations in which a particular procedure will be used. A list of essential steps follows. These can be used to mentally rehearse the procedure before it is performed. The next heading, Note these technical variations, introduces a list that is intended to prompt the surgeon for particular things to note and dictate within the template. A list of possible complications that are typically associated with that particular surgical procedure follows. Finally, a template operative dictation is provided. Common opening and terminating sequences (for example, how to enter and close the abdomen) in all laparotomy dictations are included. The volume has two primary objectives. First, it is designed to serve as a source of model operative dictations that may be individualized and used as templates. Second, it is intended as an aide-memoir, giving the surgeon a short list of pertinent information about each procedure. Ample space is provided to allow each surgeon to add notes. By reviewing this introductory material before scrubbing on a procedure, the trainee surgeon will enter the operating room better prepared to participate actively and to learn as much as possible. At the end of training, this book, with the notes accumulated by the resident, will serve as an invaluable review resource analogous to the individualized notebooks many surgeons keep.
