With the fall in mortality from infectious diseases, the· impact of childhood cancer in the Western World has increased to become the second commonest cause of death in the age group 1-14 years, being surpassed only by accidents. However, even in those countries in which paediatric cancer is attracting increasing interest, the disease is relatively uncommon, and the number of cases seen by any one physician, even in a large general hospital, is often limited to one or two a year. The widely held view by parents and even by many doctors that cancer in childhood is usually untreatable and inevitably fatal is no longer tenable. With improvement in the therapeutic response or in the actual survival rates of children with such lesions as Wilms' tumour, brain tumours, rhabdomyosarcomas, Ewing's sarcoma, retinoblastoma, lym phoma and even leukaemia, there is a real hope of achieving a substantial reduction in the mortality of childhood cancer. Paediatric oncology is, in fact, providing a vigorous stimulus to the much wider field of cancer treatment and research, and is demonstrating the advantages of a multi-disciplinary cooperation in the management of this disease.

Cancer in childhood is a rare disease, but for many reasons attracts interest out of proportion to its frequency. There is, of course, the parental anguish associated with a catastrophic illness in a son or daughter - anguish shared in large measure by the physician. This aspect no doubt has spurred on clinicians and investigators in their race to develop more effective means of treatment. The rewards are great. Not only is there the human element, but there is also the fact that survival of the successfully treated child is measured in scores of years rather than the ordinary five to ten year survival rates cited for the adult. Associated with this aspect of paediatric oncology is the responsibility to make those many years as trouble-free as possible. The challenge to the team organizing the care of children with cancer is clear. Cure is to be obtained, but not at the cost of progressively more severe somatic or psychic lesions as the child develops through adolescence to maturity. The cured child of today must not become the chronically ill adult of tomorrow. The spectrum of malignant diseases in childhood is not so varied as that of older patients. The kinds of tumour seen in children are relatively few in number; therefore, concentrated attention can be focussed on each entity, and specific strategies developed for clinical management.

Cancer is a very rare disease in children. There have been impressive gains in survival in recent years, and these have been achieved through the use of chemotherapy and national and international clinical trials. The diagnosis of cancer in a child imposes considerable stress on the whole family. In the immediate time after diagnosis, parents must learn about the disease and its treatment, explain what is happening to the child, and make arrangements for the care of other children in the family. For the child, treatment is associated with many side effects depending on the specific drugs used. In addition, the child is prone to infection and therefore is likely to miss a lot of school and other activities. For all these reasons, physicians and families have become aware that cancer has huge implications for the quality of the child's life. This book is an attempt to describe how quality of life is affected at different stages of the disease process.
