El libro aborda la difícil cuestión de saber dónde está el umbral de la normalidad. Aquel que bajo el lema de "dejar toda esperanza" delimita las fronteras de lo que convencionalmente llamamos cordura y permite analizar cuándo una mera convención social estigmatiza. El libro supone un recorrido desde los límites psico(pato)lógicos de la creatividad artística hasta las formas de entrada en la demencia. Así, determinados estados mentales de muchos creadores nos llevan a cuestionar la noción social de "normalidad", pues es una presunta anomalía la que confiere el aura al trabajo artístico, literario o filosófico. Desde una perspectiva psiquiátrica se plantea el tema de estas manifestaciones en patobiografías -los casos de Santa Teresa o de Nietzsche son ilustrativos- y se analiza el sutil umbral que configuran una serie de síntomas. Es el caso de la extrañeza, apatía o disociación. Todos ellos pueden ser considerados síntomas blandos (bisagra), pero entrañan un gran interés clínico. A ello hay que añadir el papel que juega la conciencia o insight en la enfermedad, el continuum de determinados fonemas alucinatorios, amén de la consideración de determinados síndromes como el de Munchausen o las connotaciones médico-legales de casos clínicos paradigmáticos.
