Racial and ethnic disparities in health care are known to reflect access to care and other issues that arise from differing socioeconomic conditions. There is, however, increasing evidence that even after such differences are accounted for, race and ethnicity remain significant predictors of the quality of health care received. This work covers the pathophysiology of cancer, exploring the difficulty of optimal treatment due to the complexity and diversity of cancer types. The search for distinctive molecular biology characteristics of tumor cells is especially relevant in the identification of overexpressed receptors and proteins that can be used as a target for cancer treatment. We highlight the main therapeutic modalities, particularly conventional systemic chemotherapy, addressing its mechanisms of action, therapeutic classes and even the toxic effects. We also describe the main tumor markers, their importance in the diagnosis and treatment of cancer, and the specificity of tumor cells. The first chapters serve as an introduction to the central topic of this book, targeted therapy. Key aspects of target therapy, such as classes of drugs, immunotherapy, monoclonal antibodies, checkpoint inhibitors, cancer vaccines and tyrosine kinase inhibitors are presented, and, for each one, the benefits, as well as the adverse effects are reported. Method: A qualitative descriptive study, developed with women with breast cancer undergoing some form of cancer treatment, receiving care at a Center for High Complexity in Oncology - CACON a hospital in the northwest of the state of Rio Grande do Sul. To collect data we used open interview. Results: There were difficulties in impaired self-image, side effects, prejudice, social rejection and physical limitations, which aroused feelings of sadness, hopelessness, despair, but the maintenance of life prevails in the face of difficulties and these are support in God. Conclusion: These results enable assist health professionals and families in coping with these difficulties, because they need professional support, family and friends

Cancer patients typically experience significant side-effects as the result of treatment. These side-effects can include fatigue, decreased activities of daily living (ADL) performance, and decreased quality of life (QOL). Despite the interrelationship between ADL performance and QOL, attention to ADL has been limited in the area of cancer rehabilitation. The purpose of this study was to determine the effects of a specifically designed therapeutic exercise regimen on selected functional performance tasks, perceived fatigue, and QOL measures in patients undergoing treatment for cancer. We hypothesized that exercising subjects would experience significantly less decrement in fatigue, ADL performance, and QOL than subjects not participating in structured exercise. Participants in the study were thirty male and female cancer patients (middle aged to elderly) undergoing treatment for cancer (surgery and adjuvant treatment: chemotherapy or radiation). Subjects completed questionnaires regarding fatigue, QOL, and physical activity level and were tested on various functional activities at four distinct time points: (1) at diagnosis; (2) post-surgery; (3) at 9 weeks post-diagnosis; (4) at 9 weeks post-diagnosis. Adjuvant treatment commenced between measurement point 2 and 3. Experimental subjects took part in a 17-week exercise intervention (low to moderate intensity level; commenced at diagnosis) that targeted muscle group and energy systems involved in ADL (low to moderate intensity level), while control subjects did not participate in structured exercise. A factor analysis allowed for the creation of new variables that were the combination of original dependent variables that loaded well (correlated) together. Repeated-measures ANOVA analyses revealed significant improvement in the experimental group on all study variables and significant decline in the control group. These findings indicate that participation in a specifically designed exercise intervention during treatment can elicit improvement in fatigue, QOL, and the performance of functional activities. The results also indicate that a sedentary lifestyle during treatment exacerbates treatment-related side-effects. Cancer patients should be encouraged to take part in a structured exercise program from the point of diagnosis through the end of treatment in order to improve treatment tolerance, shorten recovery time, and maintain or improve quality of life.

Since the development of pharmacoconvulsive therapy in 1934 and of electroconvulsive therapy (ECT) in 1938, ECT has proven far more valuable than just the intervention of last resort. In comparison with psychotropic medications, we now know that ECT can act more effectively and more rapidly, with substantial clinical improvement that is often seen after only a few treatments. This is especially true for severely ill patients -- those with severe major depression with psychotic features, acute mania with psychotic features, or catatonia. For patients who are physically debilitated, elderly, or pregnant, ECT is also safer than psychotropic medications. The findings of the American Psychiatric Association (APA) Task Force on ECT were published by the APA in 1990 as the first edition of The Practice of Electroconvulsive Therapy, inaugurating the development of ECT guidelines by groups both within the United States and internationally. Since then, advances in the use of this technically demanding treatment prompted the APA to mandate a second edition. The updated format of this second edition presents background information followed by a summary of applicable recommendations for each chapter. This close integration of the recommendations with their justifications makes the material easy to read, understand, and use. To further enhance usability, recommendations critical to the safe, effective delivery of treatment are marked with the designation "should" to distinguish them from recommendations that are advisable but nonessential (with the designations "encouraged," "suggested," "considered"). In the nearly two decades of cancer research in the United States, there has been little emphasis on the experiences of young adult women with breast cancer. I therefore adopted a qualitative multicase study method to explore the experiences of five young adult women from the time of discovery of a breast mass through the first three months of recovery from mastectomy. Utilizing data collected via semi-structured interviews, this study sought to describe their concerns, the way in which their concerns changed over time, and the influence of contextual factors. Three themes were identified. These included: "I'm going to die!" reassessing self, and seeking help from others. The findings identified areas for health care reform. These reforms include areas dealing with increasing the awareness of breast cancer in young adult women, facilitating early breast cancer detection. minimizing delays in treatment, and making adjustments in care such as improvements in education, early referral to the American Cancer Society, follow-up care, and the provision of individualized care to meet differing cultural needs. These "action areas" can be used to guide funding and the allocation of scarce resources for young adult women undergoing treatment for early breast cancer. Resource allocation is a topic of increasing importance in the latter part of this decade as the health care industry continues its pivotal shift from hospital centered care to managed care.
