Patient safety and quality improvement in health care remain a global priority. The Institute of Medicine's Forum on the Science of Health Care Quality Improvement and Implementation held a workshop on January 16, 2007, in Washington, DC. The workshop had its roots in an earlier forum meeting when forum members discussed what is meant by the terms "quality improvement" and "implementation science" and became convinced that they mean different things to different people. At the time, the members also discussed the need to identify barriers to quality improvement research and to implementation science. Thus the purpose of this workshop was to bring people together from various arenas to discuss what quality improvement is, and what barriers exist in the health care industry to quality improvement and also to research about quality improvement. The summary that ensues is thus limited to the presentations and discussions during the workshop itself. We realize that there is a broader scope of issues pertaining to this subject area but are unable to address them in this summary document. The workshop's first session was devoted to experiences that various institutions have had with quality improvement. Recognizing the wealth of experiences available outside of health care services, the workshop included presenters from outside the health care service industry as well as from inside. Minorities and indigenous peoples are among the most marginalized in terms of access to social and economic rights, and this is especially the case with health care. This report uses the availability, affordability, accessibility, adequacy and appropriateness framework to assess health services available to minority and indigenous communities in Egypt, Iraq and Tunisia. It combines research with interviews and focus groups with members of the communities and medical professionals to review the three health care systems. Covering the Coptic minority in Egypt; Yezidis in Sinjar in north-west Iraq; and, in Tunisia, the Black community in Djerba, Gabes and Sfax, the Jewish community in Djerba and the Amazigh community in Tatouine, the report identifies barriers to access to health care in a context of public health systems that have been weakened by Covid-19, as well as poor systems of governance and under-resourcing.

Abstract: Research has shown that patients with limited-English proficiency experience lower quality health care, have more difficulty communicating with their physician and more likely to need a translator which potentially lead to significant adverse health outcomes. The objective of this study was to demonstrate that patients with limited-English proficiency face more barriers which create equity and disparity in understanding their physician as compared to their English-speaking counterparts. Secondary data analysis was performed using the California Health Interview Survey (CHIS) 2011-2012. The CHIS is a population-based random-digit dial telephone survey conducted every two years of California's population. The survey consists of extensive questions in a variety of health related topics. For purposes of this study, specific questions were selected for statistical analysis. All the literature and data analyses strongly supported that patients with limited-English proficiency are more likely to face barriers in obtaining quality care throughout the health continuum.
