This paper discusses the issues involved in the measurement of unmet need for contraception (the "KAP-gap") and provides a new measuring model that removes the shortcomings of existing approaches. The paper reviews the existing measures of the KAP-gap, which generally define unmet need as the proportion of married women who want no more children but are not using contraception, and points out flaws in these approaches. The main shortcomings of the basic approaches include the exclusion of the following 2 groups: 1. Women who wish to stop childbearing but who may have good reasons for not practicing contraception at the time of the survey, such as the fact that they may be infecund or pregnant at the time; and 2. Women who wish to postpone childbearing but who are not implementing this spacing preference because they are not practicing contraception. By addressing these 2 shortcomings, the author's new methodology provides a more accurate picture of unmet need. The author uses data from Demographic and Health Surveys (DHS) conducted in 15 developing nations during the 1980s to show the difference between the conventional approaches and the alternative method. Previous studies of the KAP-gap have differed widely in their assessment of the size and implications of the phenomenon; some have even claimed that no unmet need exists. The new method, however, reveals an average total unmet need of 17% among the currently married women of the populations studies. Noting that the rest of the developing world probably has a roughly equal KAP-gap, the author concludes that the total number of couples or unmarried individuals in the Third World with unmet need for contraception is about 100 million.

The concept of 'unmet need for contraception', which refers to the proportion of women who do not want to become pregnant but are not using contraception, has been used in the international population field since the 1960s. The concept was developed from the first family planning and fertility surveys conducted in developing countries, which found a disconnect between women's knowledge, attitudes, and practices (KAP) about contraception. Unmet need for contraception is one of several frequently used indicators for monitoring of family planning programs, and was recently added to the millennium development goal of improving maternal health. Some other indicators that are used in combination with unmet need are the contraceptive prevalence rate (CPR), the method mix, sources of contraceptive supplies, and reasons for not using contraception. In this note, authors deal mainly with the unmet need indicator, but believe that other indicators should also be part of monitoring and evaluation of family planning programs to broaden the understanding of the use of family planning in countries.

Married women wanting no more children, who are exposed to the risk of pregnancy and are not using contraception, are defined as having unmet need for contraception for limiting purposes. Married women seeking to delay unwanted pregnancies, who are exposed to the risk of pregnancy, but are not using contraception, have unmet need for contraception for spacing purposes. This paper reviews procedures for calculating unmet need and assesses measurement errors, and their consequences, for estimating the number of women who would benefit from increased availability of modern contraceptive methods and for estimating the potential demand for such methods. High and low estimates of unmet need for contraception for purposes of limiting family size are presented for rural and urban areas of thirty-six developing countries and estimates of unmet need for spacing purposes for nine countries. Low estimates assume that women who want no more children or want to delay births for more than one year and have been breastfeeding for less than one year or are using traditional methods of contraception such as withdrawal or rhythm do not have unmet need. High estimates count these women as having unmet need.
