Abstract: Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN) is a predominantly drug-induced disease, with a mortality rate of 15-20%, that engages the expertise of multiple disciplines: dermatology, allergy, immunology, clinical pharmacology, burn surgery, ophthalmology, urogynecology, and psychiatry. SJS/TEN has an incidence of 1-5/million persons per year in the United States, with even higher rates globally. One of the challenges of SJS/TEN has been developing the research infrastructure and coordination to answer questions capable of transforming clinical care and leading to improved patient outcomes. SJS/TEN 2021, the third research meeting of its kind, was held as a virtual meeting on August 28-29, 2021. The meeting brought together 428 international scientists, in addition to a community of 140 SJS/TEN survivors and family members. The goal of the meeting was to brainstorm strategies to support the continued growth of an international SJS/TEN research network, bridging science and the community. The community workshop section of the meeting focused on eight primary themes: mental health, eye care, SJS/TEN in children, non-drug induced SJS/TEN, long-term health complications, new advances in mechanisms and basic science, managing long-term scarring, considerations for skin of color, and COVID-19 vaccines. The meeting featured several important updates and identified areas of unmet research and clinical need that will be highlighted in this white paper

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe cutaneous adverse drug reactions (SCARs). The most common causative drugs of SJS/TEN are allopurinol, carbamazepine, abacavir, phenytoin, and lamotrigine. SJS/TEN are categorized based on the percentage of epidermal detachment area: (i) SJS: less than 10%, (ii) TEN: greater than 30%, (iii) and overlapping SJS/TEN: 10,Äì30%. The pathogenesis of SJS/TEN is not fully understood, but some immunological and genetic factors are believed to be involved. There is a strong association between some specific HLA haplotypes and drug-induced SJS/TEN, for example, HLA-B*15:02 and carbamazepine-, HLA-B*58:01 and allopurinol. CD8+ cytotoxic T cells and natural killer (NK) cells play an important role in the pathogenesis of SJS/TEN, and upon the activation, they produce cytokines, chemokines, and cytotoxic proteins, that cause extensive keratinocytes apoptosis. Systemic corticosteroid and cyclosporine are still used as the first line in the treatment of SJS/TEN, in combination with care support.

Objectives: Stevens Johnson syndrome (SJS), toxic epidermal necrolysis (TEN) and their overlap (SJS/TEN) are severe acute mucocutaneous diseases, triggered manly by a newly administered drug, life threatening and rare, which still lack a definite treatment method. Our aim was to describe the clinical aspects and etiologic factors of SJS, SJS/TEN and TEN in order to evaluate treatment responses and different outcomes in mortality regarding the therapeutic options.Materials and Methods: This is a retrospective study of patients with the diagnosis described above, who were admitted to our center between 2000 and 2016 in order to characterize their clinical evaluation, treatment and mortality. Detached BSA% was used to categorize the disorder. Logistic regression was used to identify predictive variables of mortality.Results: The final diagnosis was 16 patients with SJS, 35 patients with TEN and 06 patients with SJS/TEN, with ages comprised between 15 and 91 years old, mainly females. The majority of patients with SJS were treated in the Dermatology Department and the majority of patients with TEN were treated in our Burn Center Unit. The most commonly reported causative agent was allopurinol. Oral erosion was the most reported mucosal erosion and ocular involvement was reported in 47% of patients.There was no statistical difference in mortality between patients treated with corticosteroids or intravenous immunoglobulin and plasmapheresis.ConclusionsAlthough the majority of patients with SJS can be treated outside an intensive care unit like our Burn Center Unit, TEN remains a challenging life-threatening disease and many questions regarding best treatment strategies are still without an answer. In the mean time we can state that the discontinuation of the suspected drug, wound care and early supportive therapies contribute to lowering the mortality rates.
