Inhaltsangabe:Problemstellung: In den letzten Jahren kamen die Gesundheitsorganisationen unter Druck ihre Kosten der Versorgung bei gleich bleibender oder sogar verbesserter Qualität zu reduzieren. Der Klinische Pfad stellt eine Möglichkeit dar, dieser Herausforderung gerecht zu werden. Zahlreiche Studien konnten einen positiven Effekt auf die Verweildauer, Wiederaufnahmerate, Komplikationsrate, Kosten wie auch auf die Patientenzufriedenheit nach der Einführung eines Klinischen Pfades belegen. Ziel dieser Diplomarbeit ist es, zum einen Evaluierungsmöglichkeiten für Klinische Pfade darzustellen, zum anderen den Fokus auf den Indikator „Patientenzufriedenheit“ zu legen, um die Frage zu klären, ob durch die angewendeten Instrumente zur Erhebung der Patientenzufriedenheit inhaltlich alle pfadinduzierten Änderungen abgebildet werden. Zunächst werden die Tools zur Evaluierung Klinischer Pfade identifiziert. Neben der Balanced Scorecard, dem Value Compass und dem DataMap® ist der Leuven Clinical Pathway Compass ein häufig angewendetes Instrument, welches speziell für diesen Zweck entwickelt worden ist. Um die angewendeten Instrumente zur Erhebung der Patientenzufriedenheit zu identifizieren, wurde eine systematische Literaturrecherche durchgeführt. Die geeigneten Instrumente wurden auf ihre inhaltliche Fokussierung analysiert. Dabei wurde Bezug genommen auf das Quality-of-Care-Modell, welches die möglichen Auswirkungen eines Pfades auf die Patientenzufriedenheit ausführlich dargestellt hat. Demnach ist die Erfassung der Dimensionen „Aufnahme“, „Organisation“, „Patientenorientierung“, „Information“, „Effizienz“ und „Entlassung“ von Bedeutung. Die Analyse hat ergeben, dass in vielen Studien zur Evaluation von Klinischen Pfaden selbst erstellte, nicht auf Reliabilität oder Validität geprüfte Fragebögen verwendet werden. Für die durchgeführte Untersuchung wurden zwei Instrumente mit dokumentierter Reliabilität und Validität ausgewählt und am Universitätsklinikum Salzburg bei der Implementierung des Klinischen Pfades „Leistenbruch“ parallel auf Ihre Eignung zur Evaluation der Patientenzufriedenheit bei Klinischen Pfaden getestet. Inhaltlich betrachtet sind die im Quality-of-Care-Modell geforderten Kategorien bei beiden Instrumenten inkludiert. Jedoch werden diese Dimensionen durch eine unterschiedliche Anzahl an Fragen abgefragt.
