Visceral pain, or pain in the internal organs, is often quite difficult for the clinician to recognize promptly as its characteristics are often atypical and vary with time. Patients with visceral pain also often present with concurrent painful conditions in more than one internal organ; this can give rise to an intricate complex of symptoms which can be misleading if not accurately evaluated. Visceral pain is a common occurrence that affects millions of patients world-wide every year. Although progress has been made in developing a better understanding of mechanisms underlying visceral pain and treatment alternatives, there is still a great deal that remains unknown. Visceral pain arises from various causes and often fails to respond to standard treatment, further confounding efforts to effectively prevent or treat this pain. The purpose of this text is to review current knowledge about mechanisms that contribute to pain arising from visceral structures. Inflammation plays a significant role in the pathogenesis of many forms of visceral pain, but changes in nociceptive signaling and neural structures arising from various causes clearly underlie the onset and persistence of visceral pain. Multiple processes and pathways participate in painful visceral sensations, and it appears highly probable that effective management of visceral pain may require intervention directed at several anatomic levels and signaling processes. Further, it is unlikely that a single treatment strategy will effectively manage all forms of visceral pain. It is hoped that the information contained in this text will suggest alternatives for investigation that will result in improved understanding of the pathogenesis and treatment of visceral pain.
