The clinical swallowing evaluation (CSE) is a non-instrumental exam that informs speech-language pathologists about a patient's cognition, readiness for instrumental evaluation, and swallowing symptoms. Because of the common neuroanatomy and physiology of the larynx during voicing and swallowing tasks, coughing or throat clearing after food and drink may indicate swallowing impairment. Also because of the shared mechanisms of the larynx, some clinicians also attribute voice changes after swallowing to dysphagia although many studies to date demonstrate conflicting results on the effectiveness of post-prandial voice assessment. The aim of this study was to assess if dysphonia and/or voice change after swallowing is indicative of a swallowing disorder. Thirty-nine adults between the ages of 49-97 years were audio recorded completing a sustained vowel and sentence prior to a videofluoroscopic swallowing study (VFSS) and again after swallowing each bolus. Swallowing function was categorized with a revised Penetration-Aspiration Scale, and pharyngeal residue was measured with the Normalized Residue Ratio Scale (NRRS). Two hundred and fifty voice samples were measured acoustically and perceptually. Acoustic measures of interest included mean fundamental frequency, relative average perturbation, and noise-to-harmonic ratio. Following listener training, perceptual analysis of the voice samples was completed by three speech-language pathologists using the Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V). Results indicated that baseline dysphonia was associated with greater instances of penetration and/or aspiration. Speech-language pathologists perceptually identified a change in the voice when there was a change in the acoustic signal; however, perceptually-identified voice change after swallowing was not related to dysphagia. Based on these results, dysphonia during a CSE should alert speech-language pathologists of a possible comorbid dysphagia; however, voice change after swallowing appears unrelated to airway invasion.

Introduction: Penetration and aspiration(P/A) might result in some residue in the vocal folds and alter the voice quality. These changes on the vocal folds will be reflected in the acoustic voice parameters.Purpose: The purpose of this study was to investigate feasibility of voice acoustic parameters on penetration/aspiration.Method: Twenty six patients who were taken under video fluoroscopic swallowing study (VFSS) were categorized into two groups.10 patients were included in the Non-P/A group, and 13 patients in the P/A group.A sustained vowel /a/ for at least 2 seconds was recorded before and after VFSS, and was analyzed using Multi-Dimensional Voice Program. Acoustic voice parameters included five; mean F0, relative average perturbation (RAP), Shimmer, noise-to-harmonic ratio (NHR), and voice turbulence index (VTI). Independent t tests were used to compare the differences in 5 acoustic voice parameters between the NonP/A and the P/A groups. Pre- and post-VFSS acoustic parameters within each group were evaluated with paired t test. Logistic regression was preformed to explore whether acoustic voice parameters can identify the presence of P/A.Results: In the P/A group after VFSS, the values of mF0(in male), RAP, Shimmer, NHR were increasing, but the values of mF0 (in female) and VTI were decreasing. Significant change was noted in the VTI parameter between two groups (p=0.028). Within the Non-P/A group, NHR (p=0.031) parameter had significant change after VFSS however, there was no significant difference within the P/A group. For both male and female, classification accuracy was 100% by enter method with five acoustic parameters.Conclusion: Acoustic voice analysis may be alternative evaluation for dysphagia. Also, the combination of several acoustic parameters may be screening tool for swallowing disorders.

The present study is an investigation into the association between "wet" vocal quality and prandial material in the larynx during phonation. The presence of such prandial material is indicative of oropharyngeal dysphagia and results from entry of material into the laryngeal vestibule during swallowing. The study has two primary aims: (1) to assess whether acoustic parameters can signify prandial material in the larynx during phonation; (2) to determine whether clinicians reliably perceive a "wet" vocal quality when this condition occurs. Results show that material in the larynx produces reliable acoustic consequences, including elevated measures of perturbation and signal noise and reduced amplitude of H1 relative to H2. Also, it can generate a characteristic frequency and amplitude modulation in the sound pressure waveform that is highly specific to the presence of material in the larynx during phonation. Results further show that "wet" vocal quality is not reliably perceived by clinicians when material is in the larynx during phonation, and that there is a high degree of interrater variability for perceptual judgments of "wetness". Overall, acoustic information can be useful in the clinical identification of individuals with penetration or aspiration during swallowing, and it is more reliable in detecting this condition than perceptual judgments of vocal quality. Results also indicate that there is likely to be more than one voice quality percept associated with material in the larynx, and that clinician training might increase the accuracy and reliability of perceptual judgments.
