Foreign assistance by the United States is tangled with domestic politics, and perhaps this is most clear in relation to funding for health and family planning. The long arm of U.S. domestic politics has reached the intimate lives of women all over the world because it has threatened major cuts in funding to healthcare organizations in developing countries if they perform or promote abortions. This "global gag rule," so-called because to even mention abortion endangers funding, has been a hallmark of Republican administrations since it was first enacted by President Ronald Reagan. When Donald Trump reinstated and expanded the policy, there was popular uproar and a firestorm of debate. Proponents of the policy emphasize the importance of reducing the number of abortions globally and claim that the gag rule will be effective in achieving this goal. In this innovative book, Yana van der Meulen Rodgers argues that the gag rule has failed to achieve its goal of reducing abortions. Rather, the restrictive legislation is associated with higher abortion rates, and because the reduction in funding is indiscriminate there are negative repercussions across a range of health outcomes for women, children, and men. While the rhetoric in media discourse has been extreme, Rodgers provides systematic analysis of how the global gag rule affects women's reproductive health across developing regions, grounded in a conceptual framework that models the complex factors that influence women's decision making about fertility. She also traces the background to American policy, the evolution of international family planning programs, the links between contraceptive access and fertility rates, and the relationship between restrictive abortion laws and abortion rates. And because Rodgers provides a rounded perspective on factors influencing women's decisions on reproduction and abortion, she offers a constructive and cost-effective approach for U.S. family planning assistance that targets integrated reproductive health services.

A comprehensive guide for health care workers worldwide, especially in areas with few medical resources. Designed as a manual for people confronting the HIV epidemic in their communities, no medical or technical knowledge or prior training in HIV prevention and care of people with AIDS is required. Topics range from the biology of the virus to designing successful prevention programmes and writing grant proposals. Risk factors for infection are discussed and suggestions given of helpful methods for explaining them and assisting people to change their behaviour. Extensive discussions of complex medical treatments not available to 95 percent of the people in the world who have HIV are avoided, focusing instead on medical interventions available in less industrialized settings. Illustrations highlight important topics and increase the accessibility of the text. An appendix aimed at readers with medical training discusses common AIDS-related illnesses and their treatment.

“Randy Shilts and Laurie Garrett told the story of the HIV/AIDS epidemic through the late 1980s and the early 1990s, respectively. Now journalist-historian-activist Emily Bass tells the story of US engagement in HIV/AIDS control in sub-Saharan Africa. There is far to go on the path, but Bass tells us how far we’ve come.” —Sten H. Vermund, professor and dean, Yale School of Public Health With his 2003 announcement of a program known as PEPFAR, George W. Bush launched an astonishingly successful American war against a global pandemic. PEPFAR played a key role in slashing HIV cases and AIDS deaths in sub-Saharan Africa, leading to the brink of epidemic control. Resilient in the face of flatlined funding and political headwinds, PEPFAR is America’s singular example of how to fight long-term plague—and win. To End a Plague is not merely the definitive history of this extraordinary program; it traces the lives of the activists who first impelled President Bush to take action, and later sought to prevent AIDS deaths at the whims of American politics. Moving from raucous street protests to the marbled halls of Washington and the clinics and homes where Ugandan people living with HIV fight to survive, it reveals an America that was once capable of real and meaningful change—and illuminates imperatives for future pandemic wars. Exhaustively researched and vividly written, this is the true story of an American moonshot.
