The World Health Organization (WHO) has launched the PRET initiative (Preparedness and Resilience for Emerging Threats) to help countries better prepare for future pandemics. PRET focuses on improving planning for disease outbreaks by using a “mode of transmission lens” and builds on lessons learned from the COVID-19 pandemic and other health emergencies. Its initial focus is on respiratory pathogens, with the first module aimed at guiding global, regional, and national stakeholders in pandemic preparedness for respiratory diseases. A global meeting held from 24-26 April 2023 gathered over 120 participants, including WHO members and partners, to promote political support, align stakeholders on the PRET approach, and launch the first module on respiratory pathogen preparedness. The event featured discussions, presentations, and a forecasting exercise, with the goal of creating a roadmap for global preparedness. This initiative aims to drive action for better pandemic preparedness worldwide.

A global monitoring framework has been developed to support WHO’s Preparedness and Resilience for Emerging Threats (PRET) initiative, focusing on respiratory pathogen pandemic preparedness. This framework enables WHO, countries, and partners to assess functional capacities for respiratory pandemic preparedness in the areas of emergency coordination, collaborative surveillance, community protection, clinical care, and access to countermeasures. Drawing on insights from the COVID-19 pandemic, the framework provides a structured approach to tracking global progress and identifying areas for improvement to strengthen global resilience against respiratory pathogen threats. Annual progress reports will accompany the framework to showcase the global status of functional capacities needed to prepare for future respiratory pandemics.

Since its identification in the Kingdom of Saudi Arabia and Jordan in 2012, Middle East respiratory syndrome coronavirus (MERS-CoV), a zoonotic virus that can be transmitted between dromedary camels and humans, has continued to pose a significant public health, health security and economic threat to the global community. Cases of human infection, predominantly reported to the World Health Organization (WHO) from countries in the Arabian Peninsula, have been exported to all regions of the globe. MERS-CoV is one of three high-impact zoonotic coronaviruses that have emerged during the past few decades and is included – along with SARS-CoV and SARS-CoV-2 – in the WHO list of pathogens with epidemic and pandemic potential prioritized for research and development. Over the past years, the Food and Agriculture Organization (FAO), WHO and the World Organisation for Animal Health (WOAH) have regularly brought together public health and animal health representatives from affected and at-risk countries, academic scientists and subject matter experts of high-threat respiratory pathogens to enhance collaboration across all sectors worldwide. Following two global meetings held in 2017 and 2021, the meeting in November 2023 served to review the latest scientific evidence obtained since the last global consultation in 2021 and seek consensus on research priorities for MERS-CoV and other emerging coronaviruses. This report presents the meeting summary and findings.

Vaccines are powerful weapons in the fight against pandemic viruses as shown by responses to both the 2009 H1N1 influenza and the COVID-19 pandemics. However, planning for accessing, allocating and deploying vaccines in a pandemic situation is a complex endeavour, beset with multiple challenges at all levels – local, regional and global. The World Health Organization (WHO) and its partners have prepared this revised guidance document to assist countries update their national deployment and vaccination plans (NDVPs) by leveraging global learnings from past pandemic responses, including the recent COVID-19 vaccination effort. The development and testing of a NDVP would not only advance pandemic preparedness efforts but would also have benefits in terms of increasing national capabilities to manage other health emergencies which require emergency vaccination campaigns.
