The Food, Conservation, and Energy Act of 2008 directed the U.S. Dept. of Agr. to conduct a 1-year study to assess the extent of areas with limited access to affordable and nutritious food, identify characteristics and causes of such areas, consider how limited access affects local populations, and outline recommend. to address the problem. This report presents the findings of the study, which include results from two conferences of national and internat. authorities on food deserts and a set of research studies. It also includes reviews of existing literature, a national-level assessment of access to large grocery stores and supermarkets, analysis of the economic and public health effects of limited access, and a discussion of existing policy interventions. This report updates and expands [the 2009] analysis with data from the 2010 Census, the 2006-2010 American Community Survey (ACS), and an updated list of stores from 2010. The analysis in this report is important because the past decade has seen substantial changes in population characteristics such as an aging population, migration and immigration, and wide swings in the economic well-being of families amid a 3-year recession. The development and placement of food retail stores of all types have responded to market, population, and economic conditions. Our analysis estimates supermarket access for the overall population and for those without vehicles, both in low-income and moderate/high-income areas.

Efforts to encourage Americans to improve their diets and to eat more nutritious foods presume that a wide variety of these foods are accessible to everyone. But for some Americans and in some communities, access to healthy foods may be limited. This report updates population estimates of indicators of spatial access to healthy and affordable foods in the United States using population data from the 2010 Census, income and vehicle availability data from the 2006-2010 American Community Survey, and a 2010 directory of supermarkets. Spatial access to healthy and affordable food is proxied by measuring the distance to the nearest supermarket for the overall U.S. population and for sub-populations including households without vehicles, populations with low incomes, and populations that live in low-income areas. These updated data reflect recent changes in population characteristics, the economy, and the location of supermarkets. An estimated 9.7 percent of the total population, or 29.7 million people, live in low-income areas more than 1 mile from a supermarket. However, most households that are far from a supermarket have vehicles: only 1.8 percent of all households (2.1 million households) live more than 1 mile from a supermarket and do not have a vehicle. Estimated distance to the nearest three supermarkets is an indicator of the choices available to consumers and the level of competition among stores. Estimates show that half of the U.S. population lives within 2 miles of three supermarkets.

In the United States, people living in low-income neighborhoods frequently do not have access to affordable healthy food venues, such as supermarkets. Instead, those living in "food deserts" must rely on convenience stores and small neighborhood stores that offer few, if any, healthy food choices, such as fruits and vegetables. The Institute of Medicine (IOM) and National Research Council (NRC) convened a two-day workshop on January 26-27, 2009, to provide input into a Congressionally-mandated food deserts study by the U.S. Department of Agriculture's Economic Research Service. The workshop, summarized in this volume, provided a forum in which to discuss the public health effects of food deserts.

Price and affordability are key barriers to accessing sufficient, safe, nutritious food to meet dietary needs and food preferences for an active and healthy life. In this study, the least-cost items available in local markets are identified to estimate the cost of three diet types: energy sufficient, nutrient adequate, and healthy (meeting food-based dietary guidelines). For price and availability the World Bank’s International Comparison Program (ICP) dataset is used, which provides food prices in local currency units (LCU) for 680 foods and non-alcoholic beverages in 170 countries in 2017. In addition, country case studies are developed with national food price datasets in United Republic of Tanzania, Malawi, Ethiopia, Ghana and Myanmar. The findings reveal that healthy diets by any definition are far more expensive than the entire international poverty line of USD 1.90, let alone the upper bound portion of the poverty line that can credibly be reserved for food of USD 1.20. The cost of healthy diets exceeds food expenditures in most countries in the Global South. The findings suggest that nutrition education and behaviour change alone will not substantially improve dietary consumption where nutrient adequate and healthy diets, even in their cheapest form, are unaffordable for the majority of the poor. To make healthy diets cheaper, agricultural policies, research, and development need to shift toward a diversity of nutritious foods.

Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.
