he starting point for this guideline is the point at which a woman has learnt that she is living with HIV and it therefore covers key issues for providing comprehensive sexual and reproductive health and rights-related services and support for women living with HIV. In Africa, as in many parts of the world, adolescent reproductive health is a controversial issue for policy makers and programme planners. Adolescents are particularly vulnerable to HIV and AIDS and to a host of other problems such as sexually transmitted infection, unwanted pregnancy, unsafe abortions, sexual abuse, female genital mutilation and unsafe circumcision. Refugees have rarely been studied by economists. Despite some pioneering research on the economic lives of refugees, there remains a lack of theory and empirical data through which to understand, and build upon, refugees' own engagement with markets. Yet, understanding these economic systems may hold the key to rethinking our entire approach to refugee assistance. If we can improve our knowledge of the resource allocation systems that shape refugees' lives and opportunities, then we may be able to understand the mechanisms through which these market-based systems can be made to work better and turn humanitarian challenges into sustainable opportunities. This book adopts an inter-disciplinary approach, based on original qualitative and quantitative data on the economic life of refugees, in order to begin to build theory on the economic lives of refugees. It focuses on the case of Uganda because it represents a relatively positive case. Unlike other governments in the region, it has taken the positive step to allow refugees the right to work and a significant degree of freedom of movement through it so-called 'Self-Reliance Strategy'. This allows a unique opportunity to explore what is possible when refugees have basic economic freedoms. The book shows that refugees have complex and varied economic lives, often being highly entrepreneurial and connected to the global economy. The implications are simple but profound: far from being an inevitable burden, refugees have the capacity to help themselves and contribute to their host societies - if we let them

Self-care interventions are among the most promising and exciting new approaches to improve health and well-being, both from a health systems perspective and for people who use these interventions. The World Health Organization (WHO) uses the following working definition of self-care: Self-care is the ability of individuals, families and communities to promote health, prevent disease, maintain health, and cope with illness and disability with or without the support of a health worker. The scope of self-care as described in this definition includes health promotion; disease prevention and control; self-medication; providing care to dependent persons; seeking hospital/specialist/primary care if necessary; and rehabilitation, including palliative care. It includes a range of self-care modes and approaches. While this is a broad definition that includes many activities, it is important for health policy to recognize the importance of self-care, especially where it intersects with health systems and health professionals. Worldwide, an estimated shortage of 18 million health workers is anticipated by 2030, a record 130 million people are currently in need of humanitarian assistance, and disease outbreaks are a constant global threat. At least 400 million people worldwide lack access to the most essential health services, and every year 100 million people are plunged into poverty because they have to pay for health care out of their own pockets. There is an urgent need to find innovative strategies that go beyond the conventional health sector response. While "self-care" is not a new term or concept, self-care interventions have the potential to increase choice, when they are accessible and affordable, and they can also provide more opportunities for individuals to make informed decisions regarding their health and health care. In humanitarian settings, for example, due to lack of or limited health infrastructure and medical services in the crisis-affected areas, self-care could play an important role to improve health-related outcomes. Self-care also builds upon existing movements, such as task sharing, which are powerful strategies to support health systems.

Gender-based violence has a direct impact on a range of women's reproductive health problems, including adolescent pregnancy, high-risk sexual behavior, sexually transmitted diseases, neonatal and maternal mortality, and chronic pelvic pain. To facilitate integration of these two areas, the Population Council's Ebert Program on Critical Issues in Reproductive Health and the Health and Development Policy Project met together in November 1993. It has been demonstrated that women who have been sexually abused as children are at increased risk of early initiation of intercourse, multiple partners, unprotected sex, prostitution, drug and alcohol abuse, depression, and low self-esteem. Although most societies condemn incest and forced intercourse with an unmarried virgin, coerced sex within marriage or with a sexually experienced woman may be tolerated. The general paucity of research on coerced sex reflects the more general avoidance of issues of gender and power. Seminar participants identified six priority areas for research: sociocultural contexts that shape and support sexual coercion; the integration of questions on sexual violence into ongoing research on AIDS, sexuality, and reproductive health in order to gain prevalence data; the cultural meanings attached to sexual trauma; the physical and psychological consequences of sexual coercion; processes within the criminal justice, legal, media, and medical care systems that perpetuate sexual victimization; and effective interventions.
