Adrenal tumors are commonly encountered in the practice of radiology (adrenal incidentalomas). They may arise from the adrenal gland itself, either the cortex or the medulla, or they could be secondary lesions, and they may be benign or malignant. When an adrenal tumor is identified, the key clinical questions are whether it is functioning and whether it is malignant. It is vital to be familiar with imaging characteristics of adrenal tumors for appropriate diagnosis and management. Regarding functionality, although 75% of adrenal tumors are non-functioning cortical adenomas, there is a subset of these masses with important clinical consequences (i.e. primary adrenal cancer or metastases to adrenal glands, pheochromocytoma and functional tumors that secrete cortisol, aldosterone or androgens). The delayed or false diagnosis may lead to development of severe complications of excess of secreted hormones (e.g. resistant hypertension, diabetes mellitus, osteoporosis in case of hypercortisolaemia, hypertensive crisis or myocardial infarction in case of pheochromocytoma) or metastases (in case of primary adrenal cancer of malignant pheochromocytoma).

Pheochromocytoma and paraganglioma are the primary types of neuroendocrine tumors, although they are relatively rare compared to other tumors, originating from chromaffin tissue in the adrenal medulla and/or autonomic nervous system ganglia. Because they are so rare, these tumors may go undiagnosed or undetected. Associated symptoms like hypertension are disease non-specific and may not clinically present themselves due to the fact that catecholamines can convert into their biologically inactive forms in the tumor, reducing the appearance of other symptoms as well. These tumors produce excessive catecholamines, the effects of which are manifested through various cardiac-related symptoms, such as hypertension due to increased total peripheral resistance, heart attacks despite no prior history, non-cardiogenic pulmonary shock, oedema, arrhythmias, and sudden death. In addition, these tumors have been associated with pseudo-obstruction of the bowels, diabetic ketoacidosis, and multisystem crises involving lactic acidosis. The benign pheochromocytoma and paraganglioma can progress into highly malignant phenotypes many years after the initial diagnosis, though the exact mechanisms of this are poorly understood. These tumors are considered the most familial in humans, with 25% of such tumors being hereditary, and contain mutations in twenty-nine associated genes. Thus the genetic factors causing them are highly diverse, making them extremely heterogenic. This Research Topic aims to present recent advances in Pheochromocytoma and Paraganglioma through our understanding of the underlying molecular and genetic spectrum of pheochromocytoma and paraganglioma and their clinical applications, which could provide a better understanding of the disease, improve the clinical impacts/diagnosis, and therapeutics. We welcome reviews, original research, methods, as well as perspective articles.

With the relative exception of congenital adrenal hyperplasia due to 21-hydroxylase deficiency, paediatric adrenal disorders are rare diseases, usually with an insidious onset and with relevant comorbidities, needing to be properly addressed at patients’ presentation. Moreover, due to the rarity of these disorders, they are not usually recognized at presentation, leading to a potentially harmful diagnostic delay, and may be not considered by clinicians working in non-specialized centres, with poor expertise in managing rare disorders. Furthermore, pediatric adrenal disorders inevitably develop into adult adrenal disorders, requiring a specific attention during in transitional age, in which a family-centered approach should gradually be switched to a patient-centered approach, with different needs and problems to be addressed.
