Despite decades of attention on building a global HIV research and programming agenda, HIV in older populations has generally been neglected until recently. This new book focuses on HIV and aging in the context of ageism with regard to prevention, treatment guidelines, funding, and the engagement of communities and health and social service organizations. The lack of perceived HIV risk in late adulthood among older people themselves, as well on the part of providers and society in general, has led to a lack of investment in education, testing, and programmatic responses. Ageism perpetuates the invisibility of older adults and, in turn, renders current medical and social service systems unprepared to respond to patients’ needs. While ageism may lead to some advantages – discounts for services, for example – it is the negative aspects that must be addressed when determining the appropriate community-level response to the epidemic.

In 1998, approximately 30 million people worldwide were living with HIV/AIDS, about 5 million of whom became infected that year. The epidemic continues to expand, with an estimated doubling time of 10 years, making AIDS the leading infectious cause of death ahead of tuberculosis and malaria. With the development of effective antiretroviral therapies (ART) in the mid-1990s, HIV became a treatable although serious condition, and people who are adherent to HIV medications can attain normal or near-normal life expectancies. Because of the success of ART, people 50 and older now make up a majority of people with HIV in high-income countries and other places where ART is accessible. The aging of the HIV epidemic is a global trend that is also being observed in low- and middle-income countries, including countries in sub-Saharan Africa, where the greatest number of older people with HIV reside (3.7 million). While globally over half of older adults with HIV are in sub-Saharan Africa, we have little information about the circumstances, needs, and resiliencies of this population, which limits our ability to craft effective policy and programmatic responses to aging with HIV in this region. At present, our understanding of HIV and aging is dominated by information from the U.S. and Western Europe, where the epidemiology of HIV and the infrastructure to provide social care are markedly different than in sub-Saharan Africa. Aging with HIV in Sub-Saharan Africa addresses this gap in our knowledge by providing current research and perspectives on a range of health and psychosocial topics concerning these older adults from across this region. This volume provides a unique and timely overview of growing older with HIV in a sub-Saharan African context, covering such topics as epidemiology, health and functioning, and social support, as well as policy and program implications to support those growing older with HIV. There are very few published volumes that address HIV and aging, and this is the first book to consider HIV and aging in sub-Saharan Africa. Most publications in this area focus on HIV and aging in Uganda and South Africa. This volume broadens the scope with contributions from authors working in West Africa, Botswana, and Kenya. This title was first published in 2003. The rapid demographic aging of populations worldwide, and most dramatically in developing countries, will result in unprecedented increases in the absolute and relative numbers of the aged in these countries. Whilst developed economies already have the basic infrastructure in place through which to support their ageing populations, developing nations frequently do not, and it should not be assumed that their best course of action is to attempt to duplicate the supportive infrastructures of developed countries. In developing nations these may be culturally inappropriate, geographically inaccessible, economically or politically unsustainable, or all of these. Effective and sustainable support services must be designed with reference to the circumstances of the client group, and it is increasingly evident that knowledge of the lives of the aged in developing countries is currently very limited. This book aims to inform the reader on the livelihoods of elders in developing countries and to stimulate a discussion of appropriate methods of supporting them in maintaining their quality of life during and beyond the coming decades of demographic change. It does so through reporting the lives and livelihoods of the aged population of Kikole (a pseudonym), a highly impoverished village in Uganda. Individual livelihoods are explored from a lifecourse perspective, with present day quality of life being shown often to be the result of earlier enforced changes in circumstances arising in economic, social or cultural marginalization, political or physical insecurity, or macro-economic change, rather than in the physical or mental changes that may accompany advancing age.
