This work examines the causes of airplane accidents and what private and public policies are needed to improve aviation safety. It begins by examining the safety record of the United States commuter airline industry in the post-deregulation era characterized by increased emphasis by airlines on cost control and growing pressures on the air traffic control and airport system. The authors go beyond the safety of the scheduled airlines to examine the reasons for accidents in the nonscheduled and general aviation segments of the United States industry, where the bulk of fatalities occur and where airline pilots increasingly receive most of their training and experience. Decades of continuous efforts to address known hazards in the national airspace system (NAS) and to respond to issues illuminated by analysis of incidents and accidents have made commercial airlines the safest mode of transportation. The task of maintaining a high level of safety for commercial airlines is complicated by the dynamic nature of the NAS. The number of flights by commercial transports is increasing; air traffic control systems and procedures are being modernized to increase the capacity and efficiency of the NAS; increasingly autonomous systems are being developed for aircraft and ground systems, and small aircraftâ€"most notably unmanned aircraft systemsâ€"are becoming much more prevalent. As the NAS evolves to accommodate these changes, aviation safety programs will also need to evolve to ensure that changes to the NAS do not inadvertently introduce new risks. Real-time system-wide safety assurance (RSSA) is one of six focus areas for the National Aeronautics and Space Administration (NASA) aeronautics program. NASA envisions that an RSSA system would provide a continuum of information, analysis, and assessment that supports awareness and action to mitigate risks to safety. Maintaining the safety of the NAS as it evolves will require a wide range of safety systems and practices, some of which are already in place and many of which need to be developed. This report identifies challenges to establishing an RSSA system and the high-priority research that should be implemented by NASA and other interested parties in government, industry, and academia to expedite development of such a system.

Provision of air navigation services entered a new era of performance scheme. The performance scheme provides binding targets on four key performance areas of safety, capacity, environment and cost-efficiency. It is imposed that targets are fully achieved, but it is not prescribed how, this being typical for the performance based and goal oriented regulation. Those key performance areas are interlaced by proportional and inversely proportional interdependencies. The goal of this National Transportation Safety Board study was to better understand the risk factors associated with accidents that occur in weather conditions characterized by IMC or poor visibility ("weather-related accidents"). The study accomplished this goal using the case control methodology, which compared a group of accident flights to a matching group of nonaccident flights to identify patterns of variables that distinguished the two groups from each other. This methodology expands on previous Safety Board efforts that have typically concentrated on summaries of accident cases. For this study, Safety Board air safety investigators (ASI) collected data from 72 GA accidents that occurred between August 2003 and April 2004.
