In this study we proposed to verify the results of previous ASSR research by examining the accuracy of the ASSR in the estimation of pure tone thresholds in normal-hearing adults with simulated conductive hearing losses. A combination of cotton and foam was used to create a simulated conductive hearing loss in 10 normal-hearing adult subjects. Pure tone air and bone conduction audiometry as well as ASSR testing was completed on all 10 subjects. The results of the study revealed that the air conducted ASSR to repeated sequence tonal stimuli can predict behavioral pure tone simulated air conduction thresholds to within 0-5 dB nHL and 10-25 dB SPL at carrier frequencies of 500, 1000, 2000 and 4000 Hz. Similarly, bone conducted ASSRs can predict pure tone bone conduction thresholds to within approximately 5-20 dB nHL and 20-30 dB SPL at carrier frequencies of 500 Hz to 4000 Hz. Based on these results and comparable findings from previous ASSR research, audiologists should feel comfortable in relying on the ASSR to accurately predict the pure tone audiogram.

Background: The Auditory Steady State Response (ASSR) is an auditory evoked potential (AEP) that can be used to objectively estimate hearing sensitivity in individuals with normal hearing sensitivity as well as those with various degrees and configurations of sensorineural hearing loss (SNHL). Purpose: This purpose of this project is to create a tutorial on the ASSR for audiologists and AuD students who have limited (1 - 5 years) clinical experience with AEPs. It is the authors' hope that this manuscript can serve as a central resource for current information regarding the various aspects of this response. Design: This tutorial has been organized into various sections including: the history of the ASSR, unique terminology associated with this response, the types of stimuli used to elicit the response, two common stimulation methods, two common ways to objectively analyze the response and a discussion of the frequency and place specificity of this response for both air and bone conduction testing. This manuscript contains a glossary of key terms located at the end of the document. The tutorial contains numerous figures which attempt to walk the reader through key concepts associated with this response. Several tables provide a summary of the accuracy of the ASSR in estimating pure tone air and bone conduction thresholds for individuals with normal hearing sensitivity as well as those with SNHL. Topics such as the influence of degree and configuration of SNHL as well as the effects of single frequency versus multiple frequency stimulation techniques on the accuracy of estimating air-conduction behavioral thresholds via ASSR are discussed. Similarly, the effects of bone conduction oscillator placement, coupling force, the number of recording channels, and the influence of neuro-maturation are discussed for estimating bone-conduction behavioral thresholds via ASSR. Conclusion: Most researchers agree that in the future, ASSR testing will play an important role in clinical audiology across various populations. Therefore it is important for clinical audiologists and AuD students to have a good basic understanding of the technical concepts associated with this response as well as a knowledge of optimal stimulus and recording parameters used to record this response.

Abstract: Abstract of dissertation entitled Prediction of Hearing Thresholds: Comparison of Cortical Evoked Response Audiometry and Auditory Steady State Response Audiometry Techniques Submitted by Yeung, Ngan Kam Kammy for the degree of Master of Science in Audiology at the University of Hong Kong in May 2004 The aim of the present study is to evaluate how well cortical evoked response audiometry (CERA) and auditory steady state response (ASSR) thresholds predict behavioral pure tone audiometric (PTA) thresholds at octave frequencies between 500 and 4000 Hz. 34 adult subjects aged 18 or above with normal hearing or sensorineural hearing loss ranging from mild to profound level were recruited. 63 ears met the test criteria for PTA, CERA and ASSR testing. In addition to overall analyses, these ears were divided into 3 groups (normal hearing, mild to moderately severe hearing loss, and severe to profound hearing loss) according to their average PTA thresholds for individual group analyses. CERA results measured using tone burst stimuli, and ASSR results measured using 100% amplitude modulated and 10% frequency modulated tone stimuli at a modulation frequency of 40 Hz were compared with reference to PTA thresholds. Overall correlations of CERA and ASSR thresholds with PTA thresholds were high, with viii Pearson r coefficients exceeding .97 and .95 respectively for each of the test frequency. Individual group correlations, however, were significant at all test frequencies only in the hearing impaired groups. CERA and ASSR thresholds differed significantly from PTA thresholds in the normal hearing group. In addition, overall and individual group mean discrepancies between CERA and PTA thresholds were in general less than those between ASSR and PTA thresholds. 78.2% of the test ears had CERA thresholds within 10 dB of PTA thresholds and 71.0% of test ears had ASSR thresholds within 15 dB of PTA thresholds, showing that CERA thresholds better predicted PTA thresholds than ASSR thresholds. Results of the present study confirmed findings from previous research that both ASSR and CERA testing were accurate in predicting PTA thresholds for adult subjects with sensorineural hearing loss, but not as accurate for those with normal hearing. This study attempted to determine the most appropriate clinical protocol for using the Auditory Steady-State Response for the prediction of pure tone behavioral thresholds. Behavioral and Auditory Steady-State Response thresholds were measured at 500 - 4000 Hz from sixteen normal-hearing subjects. All subjects were tested using both the single-frequency ASSR test approach as well as the multiple-frequency ASSR test approach in an attempt to determine which approach was the most accurate predictor of behavioral pure-tone thresholds. Additional test parameters, such as the EEG recording channel (ipsilateral versus contralateral) and overall test duration were evaluated in order to determine the most appropriate clinical protocol for obtaining Auditory Steady-State Response thresholds. The results of this study revealed that using a combination of the multiple-frequency ASSR test approach to initially bracket threshold and the single-frequency ASSR test method to better define threshold would most likely yield the most accurate estimation of pure-tone behavioral thresholds in the most acceptable amount of time. The study also determined that a two channel recording (ipsilateral and contralateral) should be employed to yield the most sensitive ASSR thresholds.
