OBJECTIVE This study surveys patients with schizophrenia or schizoaffective disorder, to understand the relationship between sexual function and treatment with antipsychotic medication. The study compares patients on single versus multiple antipsychotics as well differences between first and second generation agents. MATERIAL AND METHOD Patients diagnosed with schizophrenia or schizoaffective disorder are eligible. Once patients are evaluated to ensure they meet enrollment criteria and are consented, they are administered the Positive and Negative Syndrome Scale (PANSS), Abnormal Involuntary Movement Scale (AIMS), and either the International Index of Erectile Function for men, or the Female Sexual Function Index for women. Inclusion criteria include age 18-65, able to participate in a structured interview, fulfill DSM-IV criteria, and on stable doses of one or more antipsychotic medications for at least six weeks. Exclusion Criteria include patients taking Selective Serotonin Reuptake Inhibitors (SSRIs), and inability to provide informed consent.RESULTS Patients taking typical antipsychotic had marginally worse score on sexual function scale. Patients taking multiple antipsychotics scored better on the sexual function scales. Those on prolactin-sparing agents tended to rate sexual function as higher. Clozapine was associated with more sexual dysfunction compared to other antipsychotics. No trend noticed between PANSS scores and sexual dysfunction. No trend noticed related to AIMS scores. CONCLUSIONS There is high prevalence of sexual dysfunction in schizophrenic patients. Prolactin sparing agents are associated with less sexual dysfunction. There is an advantage to the use of atypical antipsychotics.

Despite growing awareness in the psychiatric community of the multifaceted medical needs of the severely mentally ill, statistics show that as much as 60% of all schizophrenia patients die prematurely from nonpsychiatric medical conditions -- in part because many physicians have not yet recognized how to properly treat common diseases and illnesses within this complex patient population. Medical Illness and Schizophrenia, Second Edition, is the only clinical guide to focus exclusively on the treatment of common medical comorbidities among patients with schizophrenia. Like its best-selling predecessor, the book compiles the latest research and clinical information on integrating medical and psychiatric care for the schizophrenia patient. Twenty-eight physicians and psychiatrists, including editors Jonathan M. Meyer, M.D., and Henry A. Nasrallah, M.D., lend their expertise to this new, expanded edition. In fifteen chapters, this volume covers a wide range of common medical problems -- from metabolic and heart conditions, such as cardiovascular disease, hyperlipidemia, obesity and diabetes, to substance abuse and smoking. Each chapter concludes with "Key Clinical Points" that summarize important concepts and ensure reader retention. Additionally, the second edition includes new chapters that touch on some of the most complex clinical issues in the field of schizophrenia treatment today: Recent trends in the integration of medical and mental healthcare Behavioral treatments for weight loss in persons with schizophrenia Treatment of sexual dysfunction among persons treated for schizophrenia Health outcomes of schizophrenia treatment in children and adolescents Health outcomes of schizophrenia treatment during pregnancy and breastfeeding As the only clinical text of its kind, Medical Illness and Schizophrenia, Second Edition, is an invaluable resource for psychiatrists, nurses, healthcare professionals, and psychiatric and clinical residents. The goal of this text has always been to help clinicians recognize schizophrenia as both a brain disorder and a systemic disease with multiple manifestations that go beyond the obvious psychiatric symptoms -- and thus take a broader approach to treatment of schizophrenia. This new edition is a comprehensive, practical manual that serves as a reference for the medical management of seriously mentally ill patients across the age spectrum in both inpatient and outpatient settings.

This book addresses the fundamental importance of the proper assessment and treatment of sexual disorders in patients with mental disorders with regard to their treatment outcomes and adherence to therapies. In fact, patients with mental disorders often suffer from sexual dysfunctions and in many cases, pharmacological treatment causes sexual-dysfunction side-effects. However, anxiety disorders, personality disorders, eating disorders and psychotic disorders are often also characterized by sexual symptoms that have a profound impact on sexual function. The psychopathology of sexual behavior investigates deficits in the mind-body relationship that are expressed through sexual symptoms such as erectile dysfunction, premature ejaculation, vaginismus, orgasmic disorders, or lack of sexual desire. These symptoms can be considered both prodromal and as a consequence of psychological or psychiatric suffering. Written by a multidisciplinary team of experts, this book offers a valuable guide for psychiatrists, andrologists, gynecologists and psychologists.

Reflecting current understanding of the complexities of sexual activity among persons with chronic mental illness, the text draws upon the collective wisdom and experience of experts from a variety of settings. Clinicians, advocates, consumers, researchers, legal experts, and administrators all contribute to document the concerns about sexual behavior and the consequent health risks for this at-risk population. The research presented here is particularly timely in view of recent emphases on patient choice, recovery, and advocacy, and can be used to provide guidance to clinicians, mental health administrators, policymakers, advocates, and researchers.
