There is increasing evidence for the clinicial value of the apo lipoprotein measurements. Besides cholesterol in plasma and li poprotein fractions, which is currently used as an indicator of cardiovascular risk, the measurement of the AI and B apolipopro teins can provide additional information about the patients' clinical status. Several studies show that apo B is higher and apo AI is lower in patients with angiographically documented coronary heart dis ease than in symptomatic patients without coronary heart disease. Moreover, discriminant analysis indicated that the concentration of Apo AI and B in plasma are better discriminators than lipo protein cholesterol for identifying patients with coronary heart disease. In some studies the apo Bjapo AI ratio appears to be a more powerful predictor than individual lipoproteins. In a recent study carried out in men, apolipoproteins AI and B were better correlated with the severity of cardiovascular disease than HDL and LDL cholesterol. The predictive power of apolipoproteins could however not be demonstrated in all studies and the value of apolipoprotein measurements in the field of clinical chemistry is still controversial. This is probably due to discrepancies between the results of various studies, arising from differences in the type of immunoassays, the lack of universal reference materials, differ ences between study protocols, variations in the selection of patients and in the grading and interpretation of coronary lesions.

Low density lipoproteins (LDL) are pathophysiologically important be cause of their central role in the disease atherosclerosis and because atherosclerosis is the leading cause of death in developed countries. Many researchers believe that a more detailed knowledge of the struc ture, function, and metabolism of LDL may eventually lead to a means to control atherosclerosis. For this reason a fairly large research effort has gone into the investigation of LDL over the past few years. The purpose of this book is to collect and summarize in one place most of the pub lished information on LDL through 1975. To this end more than 1500 references are cited in the papers that make up this volume. The A, B, C apolipoprotein classification system was adopted for use throughout this work. In addition to the A, B, C, and "D" families of apolipoproteins, apoE is used to designate the "arginine-rich" apolipo protein. This classification system is used because it is far less cumber some than other proposed classification schemes for apolipoproteins.

Plasma lipoproteins constitute a unique macromolecular system of lipid-protein complexes responsible for the transport of lipids from their sites of origin to their sites of utilization either as metabolic fuel or as structural components of cell membranes. Although studies on the role of lipoproteins in the mechanism of lipid transport are meritorious in their own right, the ever-increasing interest in chemical and functional properties of this remarkable class of conjugated proteins stems from the impressive evidence of their direct involvement in the genesis and develop ment of atherosclerotic lesions. The initial emphasis on neutral lipids and phospholipids as the most characteristic constituents of operationally defined lipoprotein classes has shifted in recent years to their protein moieties or apolipoproteins. The discovery of a number of apolipoproteins and characterization of familial hypolipoproteinemias as apolipoprotein deficiency disorders indicated that apolipoproteins play an essential role in maintaining the structural stability and integrity of lipoprotein particles. In addition to their role in the formation of lipoproteins, apolipoproteins were shown to perform a variety of functions in metabolic conversion of lipoproteins and their interactions with cellular surfaces. Results from several laboratories have demonstrated that the chemical and metabolic heterogeneity of operationally-defined lipoprotein classes is due to the presence of several discrete lipoprotein particles with similar physical properties but different and characteristic apolipoprotein composition. Thus, the apolipoproteins have emerged not only as essential structural and functional constituents of lipoproteins but also as unique chemical markers for identifying and classifying lipoprotein particles.

For the past 30 years I have been teaching lipid biochemistry to'inedical students, graduate students, and undergraduate students. The major topics covered in my courses were fatty acids, prostaglandins, leukotrienes, phospholipids, glyco lipids, triacylglycerols, cholesterol, bile acids, and plasma lipoproteins. Empha sis was placed on the regulation and disorders of lipid metabolism. The latter included hyperlipidemias, atherosclerosis, and alcohol-induced liver damage. In this volume, I have chosen to focus on the disorders of lipid metabolism at a level appropriate both for medical students and for graduate and undergradu ate students majoring in the biological sciences. The biochemistry, nutrition, genetics, and cell biology aspects of lipids and lipid metabolism will be covered as they relate to lipid disorders. I am not aware of any textbook that integrates the disorders of lipid metabolism in this manner. Chapter 1 includes a brief discussion of the basic structures, properties, and metabolism of lipids. This chapter is not very detailed, since the material covered is available in basic textbooks on biochemistry. The major fpcus of this volume is the various lipid disorders, with emphasis on polyunsaturated fatty acids, the molecular biology and pathogenesis of the hyperlipidemias, dietary and drug therapy for the hyperlipidemias, and alcohol-induced liver damage. The material presented has been obtained from several textbooks on biochemistry and from a variety of recent articles in the scientific literature.

The Proceedings of the Eight International Symposium on Drugs Affecting Lipid Metabolism (8th D.A.L.M.) is the subject of this volume. Since the first symposium in 1960, each successive meeting has broken new ground in the field of pharmacological control of lipid levels - offering new and stimulating insights and exposing the audience to the state of the art. The field has progressed sufficiently to permit discussion of the cellular biology of athero sclerosis. The opening session was devoted to pathology, macrophages, lipoproteins and their receptors and choles terol ester metabolism. Because of the recent emergence of new apolipoprotein technology, a workshop devoted sole ly to apolipoprotein methodology was introduced followed by a plenary session devoted to their metabolism and structure. Another rapidly developing area of atherosclerosis research is non-invasive assessment of this condition. Accordingly, a session was devoted to new techniques for this research modality. The final plenary sessions were devoted to the roles of drugs and diet in athero~ scl~rosis - cause, treatment and mechanisms of action. The meeting was summarized by Dr. O.J. Pollak, one of the "founding fathers" of this field. There were nine sessions of proffered papers whose abstracts appear in this volume. In addition, special workshops (to be reported elsewhere) were devoted to several drugs including Oiyzanol, Probucol and Etofibrate.
