Os fluxos migratórios são normalmente identificados como um desafio de saúde pública, assumindo-se a importância (e a necessidade) de compreender os impactos das migrações na saúde, tanto na perspetiva dos sistemas de saúde dos países de acolhimento de imigrantes, como na perspetiva das populações imigrantes e não imigrantes residentes nesses contextos. O melhor conhecimento da saúde dos imigrantes e do seu acesso e utilização do sistema de saúde é essencial para as políticas de integração e de saúde de um país, sendo porém desafiado pela falta de dados disponíveis na maioria dos países europeus. Apesar do crescente reconhecimento da importância deste tema - patente tanto em recomendações e avaliações de organizações internacionais; no desenvolvimento de políticas e programas de intervenção para a saúde das populações migrantes; e no aumento da investigação e estudo neste domínio – persistem lacunas no conhecimento da relação entre migrações e saúde, e dos reais impactos das migrações na saúde. Este segundo Caderno Estatístico, da Coleção Imigração em Números do Observatório das Migrações, procura responder exatamente a estas lacunas, sistematizando e analisando informação estatística de várias fontes (nacionais e internacionais), para o período de referência de 2005 a 2016, que permitem retratar, de forma comparada os imigrantes e os nativos, quanto ao estado de saúde - a partir da autoapreciação do estado de saúde e da qualidade de vida, do reporte de incapacidades temporárias por problemas de saúde, e de doenças crónicas -, à acessibilidade e utilização dos serviços de saúde – confrontando as estatísticas da utilização de serviços de saúde, com o enquadramento legal e institucional do acesso à saúde e as barreiras de acesso e efeitos desmobilizadores do uso dos serviços de saúde -, e à mortalidade e causas de morte. A análise dos dados disponíveis induz à identificação de algumas iniquidades em saúde na comparação dos imigrantes com os não imigrantes em Portugal, sendo essas desigualdades enquadradas pelos determinantes da saúde, na sua dimensão estrutural, social e grupal, e individual. Assumindo que a relação entre migrações e saúde tem sido estabelecida de forma parcelar, as autoras caracterizam ainda para a última década a evolução desta relação atendendo a três universos que integram o mesmo fenómeno: (1) o universo de fluxos de entrada e de saída por razão de saúde, atendendo à articulação e cooperação internacional portuguesa na vertente da saúde; (2) o universo de imigrantes residentes que, em virtude da sua permanência e integração no país, necessitam de proteção de saúde; e (3) o universo de profissionais de saúde estrangeiros que integram o sistema de saúde português. O Caderno traz, assim, numa terceira vertente, a análise de dados que sustentam também a leitura dos contributos dos imigrantes para o sistema de saúde português.
