A vast literature has been concerned with arteriosclerosis and yet, many aspects of pathogenesis and of the mechanism of development of the arteriosclerotic vascular lesion remain only poorly understood. In recent years, our knowledge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. It is also because of the development of new drugs that interfere with the atherogenic process and thereby prevent the development of the disease or halt its progression. It is also becoming increasingly possible to inhibit the occurrence of complications in existing arteriosclerotic lesions in manifest disease, i. e.

Substantial progress toward reconciling disparate theories of the pathogenesis of arteriosclerosis was recorded in Volume I of this book. the proceedings of an interdisciplinary workshop conference held at Lindau. Germany, April 19-25, 1970. Strong evidence was ad duced that the early stages of arteriosclerosis consist of intimal proliferation similar to that associated with the increase in arteri al size and caliber that characterizes normal growth. The findings described recall a theory proposed by Richard Thoma (Thoma and Kaefer, 1889) of Heidelberg toward the end of the nineteenth century. He sug gested that the pathogenesis of arteriosclerosis began with intimal thickening that was initially adaptive. Past a certain point, how ever, the thickening compromised the nutrition of the artery wall, leading inevitably to degenerative changes. In 1944 Hueper (Hueper, 1944), reemphasized the threat to the nu tritional support of the artery and proposed that the many etiologic factors capable of inducing arterial atheroma did so through the final common pathway of interfering with oxidative mechanisms in the vessel wall. Thus, it is proposed that arteriosclerosis is basically an aspect of the "behavior" of the artery. The nature of the lesion, therefore, depends more on the biochemical and structural response capabilities of the tissue itself than on the characteristics of the various etiologic agents. The idea that arteriosclerosis begins as uncontrolled or disturbed adaptive behavior was shared by Duff and also by Winternitz (Duff, 1954; Winternitz, 1954).

Presently, and in the past, the predominant investigative emphasis among research workers in arteriosclerosis has been on plasma and arterial lipids. Recent data from a number of laboratories suggest that arterial mesenchyme is of considerable importance in the pathogenesis and fate of arteriosclerotic lesions. The significance of some of these observations made it clear that there was need for intensified research on the connective tissue components of the arteriosclerotic lesion and that arteriosclerosis research workers could benefit from a more comprehensive view of the subject. Because of their experience in the field of arteriosclerosis and their interest in stimulating new directions for research on the lesion, the Committee on Coronary Artery Lesions and Myocardial Infarctions of the Council on Arteriosclerosis, American Heart Association, planned an International Workshop on Arterial Mesenchyme and Arteriosclerosis. The Workshop brought together scientists expert in connective tissue research and research on arteriosclerosis who presented the current status of knowledge in their areas of expertise. The Workshop was held April 2-3, 1973 at the Royal Orleans Hotel, New Orleans, Louisiana and was attended by more than 170 people. The twenty papers and discussions presented in this volume summarize the proceedings of the Workshop and represent a comprehensive review of the role of arterial mesenchyme in arteriosclerosis.
