There has always been a need for objective tests that assess auditory function in infants, young children, and/or any patient whose development level precludes the use of behavioral audiometric techniques. Although the Auditory Brainstem Response (ABR) is seen as the gold standard in the field of objective audiometry, it presents with its own set of limitations. The Auditory Steady State Response (ASSR) has gained considerable attention and is seen as a promising addition to the AEP family to address some of the limitations of the ABR. The ASSR promises to estimate all categories of hearing loss (mild to profound) in a frequency specific manner. It also indicates to the possibility to validate hearing aid fittings by determining functional gain of hearing aids by determining unaided and aided ASSR thresholds. An exploratory research design was selected in order to compare unaided thresholds, obtained through the use of three different procedures ABR, ASSR and behavioral thresholds. Aided thresholds were also obtained and compared with two procedures the aided ASSR (measured and predicted) and aided behavioral threshold. The results indicated that both the ABR (tone burst and click) and ASSR provided a reasonable estimation of the subsequently obtained behavioral audiograms. The ASSR, however, approximated the behavioral thresholds closer than the ABR and were furthermore able to quantify hearing thresholds accurately for subjects with severe and profound hearing losses. The result indicated further that the ASSR can be instrumental in the validation process of hearing aid fittings in infants. These results demonstrated however, that the ASSR measured thresholds underestimate the aided behavioral thresholds and the aided ASSR predicted thresholds overestimate the aided behavioral thresholds. The research concluded that the ASSR is useful in estimating frequency-specific behavioral thresholds accurately in infants and validating hearing aid fittings. Until evidence is sufficient to recommend the ASSR as primary electrophysiological measure of hearing in infants, the ASSR should be used in conjunction with the ABR following a test battery approach in the diagnostic process of hearing loss in infants. The ASSR further shows great promise in validating hearing aid fittings, but this specific application of the ASSR needs further research evidence on large groups to validate the procedure.

Abstract: The ability to obtain frequency specific information regarding a patient's hearing sensitivity in an objective manner allows the evaluation of patient populations who cannot be tested through traditional behavioral methods. One method for obtaining this information is the auditory steady state response (ASSR). ASSR permits the testing of multiple carrier frequencies simultaneously and both ears simultaneously, unlike the auditory brainstem response (ABR). ASSR replaces subjective examiner interpretation of the response with statistical analyses not subject to the variability of human observers. Unlike ABR which has been in use for decades and utilizes relatively consistent stimuli and test protocols, the ASSR has only been in widespread clinical use for the past 6-8 years and consequently does not have the same level of standardization as ABR. ASSR can be elicited by a variety of stimulus types including but not limited to: (1) Sinusoidal amplitude modulated (SAM) tones, (2) Frequency modulated (FM) tones, (3) Mixed modulation (MM) tones, and (4) Toneburst (TB) trains. The ASSR response is found in the frequency domain at the frequency of modulation and is frequently differentiated from unrelated neural activity using an F-statistic to determine if the amplitude of the line spectra at the modulation frequency is statistically different from the surround physiologic noise. The current study sought to evaluate several common stimuli used in ASSR testing to determine if a more recently introduced stimulus (TB) emerges as a more appropriate stimulus for generating the response. Response detection and collection parameters were standardized so that any differences seen could be attributed to the stimulus. The Auditory Steady State Response (ASSR) procedure has been established as a frequency specific, objective audiologic measure, which can provide reliable thresholds to within 10 dB of the behavioral thresholds. In order for ASSR to find its place in the existing framework of audiometric procedures, the full potential of the procedure needs to be explored. The aim of this study was to determine the accuracy of monotic ASSR in estimating hearing thresholds in a group of 15 normal hearing subjects and 15 hearing-impaired subjects. A comparative research design was implemented. Indicating that results obtained in the study was compared to relevant literature where dichotic multiple ASSR was implemented. This was done in order to ascertain ASSR's capabilities with regard to stimulus presentation methods. Monotic single ASSR predicted behavioural thresholds in the normal hearing subjects within an average of 24 dB across the frequency range (0.5, 1, 2 & 4 kHz). In the hearing-impaired group, ASSR thresholds more closely resembled behavioural thresholds, with an average difference of 18 dB, which is consistent with recent literature. The literature suggests that better prediction of behavioural thresholds will occur with greater degrees of hearing loss, due to recruitment. The focus in this group also centered on the accurate prediction of the configuration of the hearing loss. It was found that ASSR could reasonably accurately predict the configuration of the hearing loss. In the last instance, monotic single and dichotic multiple ASSR were compared with regard to threshold estimation and prediction of configuration of the hearing loss in the hearing-impaired group. Little difference was reported between the two techniques with regard to the estimation of thresholds in both the normal hearing and hearing impaired groups. In conclusion it was established that monotic ASSR could predict behavioural thresholds of varying degrees and configurations of hearing loss in normal and hearing-impaired subjects with a reasonable amount of accuracy. At this stage, however, more research is required to establish the clinical validity of the procedure, before it is routinely included within an objective test battery.

Abstract: Abstract of dissertation entitled Prediction of Hearing Thresholds: Comparison of Cortical Evoked Response Audiometry and Auditory Steady State Response Audiometry Techniques Submitted by Yeung, Ngan Kam Kammy for the degree of Master of Science in Audiology at the University of Hong Kong in May 2004 The aim of the present study is to evaluate how well cortical evoked response audiometry (CERA) and auditory steady state response (ASSR) thresholds predict behavioral pure tone audiometric (PTA) thresholds at octave frequencies between 500 and 4000 Hz. 34 adult subjects aged 18 or above with normal hearing or sensorineural hearing loss ranging from mild to profound level were recruited. 63 ears met the test criteria for PTA, CERA and ASSR testing. In addition to overall analyses, these ears were divided into 3 groups (normal hearing, mild to moderately severe hearing loss, and severe to profound hearing loss) according to their average PTA thresholds for individual group analyses. CERA results measured using tone burst stimuli, and ASSR results measured using 100% amplitude modulated and 10% frequency modulated tone stimuli at a modulation frequency of 40 Hz were compared with reference to PTA thresholds. Overall correlations of CERA and ASSR thresholds with PTA thresholds were high, with viii Pearson r coefficients exceeding .97 and .95 respectively for each of the test frequency. Individual group correlations, however, were significant at all test frequencies only in the hearing impaired groups. CERA and ASSR thresholds differed significantly from PTA thresholds in the normal hearing group. In addition, overall and individual group mean discrepancies between CERA and PTA thresholds were in general less than those between ASSR and PTA thresholds. 78.2% of the test ears had CERA thresholds within 10 dB of PTA thresholds and 71.0% of test ears had ASSR thresholds within 15 dB of PTA thresholds, showing that CERA thresholds better predicted PTA thresholds than ASSR thresholds. Results of the present study confirmed findings from previous research that both ASSR and CERA testing were accurate in predicting PTA thresholds for adult subjects with sensorineural hearing loss, but not as accurate for those with normal hearing. This study attempted to determine the most appropriate clinical protocol for using the Auditory Steady-State Response for the prediction of pure tone behavioral thresholds. Behavioral and Auditory Steady-State Response thresholds were measured at 500 - 4000 Hz from sixteen normal-hearing subjects. All subjects were tested using both the single-frequency ASSR test approach as well as the multiple-frequency ASSR test approach in an attempt to determine which approach was the most accurate predictor of behavioral pure-tone thresholds. Additional test parameters, such as the EEG recording channel (ipsilateral versus contralateral) and overall test duration were evaluated in order to determine the most appropriate clinical protocol for obtaining Auditory Steady-State Response thresholds. The results of this study revealed that using a combination of the multiple-frequency ASSR test approach to initially bracket threshold and the single-frequency ASSR test method to better define threshold would most likely yield the most accurate estimation of pure-tone behavioral thresholds in the most acceptable amount of time. The study also determined that a two channel recording (ipsilateral and contralateral) should be employed to yield the most sensitive ASSR thresholds.

Background: The Auditory Steady State Response (ASSR) is an auditory evoked potential (AEP) that can be used to objectively estimate hearing sensitivity in individuals with normal hearing sensitivity as well as those with various degrees and configurations of sensorineural hearing loss (SNHL). Purpose: This purpose of this project is to create a tutorial on the ASSR for audiologists and AuD students who have limited (1 - 5 years) clinical experience with AEPs. It is the authors' hope that this manuscript can serve as a central resource for current information regarding the various aspects of this response. Design: This tutorial has been organized into various sections including: the history of the ASSR, unique terminology associated with this response, the types of stimuli used to elicit the response, two common stimulation methods, two common ways to objectively analyze the response and a discussion of the frequency and place specificity of this response for both air and bone conduction testing. This manuscript contains a glossary of key terms located at the end of the document. The tutorial contains numerous figures which attempt to walk the reader through key concepts associated with this response. Several tables provide a summary of the accuracy of the ASSR in estimating pure tone air and bone conduction thresholds for individuals with normal hearing sensitivity as well as those with SNHL. Topics such as the influence of degree and configuration of SNHL as well as the effects of single frequency versus multiple frequency stimulation techniques on the accuracy of estimating air-conduction behavioral thresholds via ASSR are discussed. Conclusion: Most researchers agree that in the future, ASSR testing will play an important role in clinical audiology across various populations.
