The book Autoimmune Rheumatic Disease (second edition) is a new fully revised edition of the award winning title. It fills the gap in the literature in that no other book bridges the divide between the clinical characterisation and treatment of autoimmune rheumatic diseases on the one hand and an understanding of laboratory-based research and disease pathogenesis on the other. This second edition is especially important because it describes and explains the advances in molecular biological techniques that have brought about major changes in understanding and also covers the new therapies which have been developed for many autoimmune rheumatic diseases.

The prevalence of autoimmune diseases and rheumatic conditions is constantly increasing. Autoimmune diseases affect approximately 7-10% of the population of the United States, while more than 50,000,000 American adults suffer from some type of arthritis. The Heart in Rheumatic, Autoimmune and Inflammatory Diseases examines the complex mechanisms relating to cardiac diseases from a pathophysiological and clinical point of view. Autoimmune rheumatic diseases can affect the coronary vessels, myocardium, pericardium, heart valves and the conduction system. The diagnosis of these unique cardiac complications necessitates medical awareness and a high index of suspicion. Increased risk of advanced atherosclerosis plays a pivotal role in the development of cardiac diseases in systemic, rheumatic and autoimmune illnesses. Yet, other complex immune medicated mechanisms may contribute to the pathogenesis. Patients' optimal care requires coordination between the primary caregiver, the rheumatologist, immunologist and cardiologist. Screening for cardiovascular risk factors, recognition of high-risk patients and identification of subclinical cardiac conditions are of great importance. Moreover, regulation of inflammation, as well as abnormal immune responses and the initiation of early treatments should be the focus of patient management. A continuous attempt to identify novel therapeutic targets and change the natural history of the underlying disease and its cardiac manifestations is in progress. The book aims at providing the readers with a state of the art collection of up to date information regarding clinically important topics based on experts' perspectives. This book was a result of an extended coordinated collaboration of one-hundred and fifty-four distinguished scientists from thirty-one countries around the globe. A review of common, as well as unusual (yet clinically significant) medical cardiac complications of prevalent rheumatic, autoimmune and inflammatory diseases. Focuses on aspects of pathophysiological processes, clinical presentations, screening tests, prognostic implications and novel therapeutic approaches. Presents an up-to-date “level of evidence and “strengths of recommendations for suggested therapies and reviews all randomized clinical trials, meta-analyses and other supporting published clinical findings.

Immunology and Rheumatology in Questions, 2nd Edition addresses through short and concise questions-and-answers (Q&As) on one hand all major aspects of basic clinical and laboratory immunology necessary for understanding underlying immunological mechanisms of autoimmune rheumatic diseases. The majority however of Q&As in this book presents in a laconic way definitions, pathogenetic aspects, clinical and laboratory manifestations, differential diagnosis and the management of all categories of rheumatic diseases including systemic autoimmune, autoinflammatory, metabolic and degenerative. Furthermore, in separate sections of this manual Q&As addressing rheumatic manifestations from other organ systems are included. Finally, a chapter is devoted to treatment of rheumatic diseases analyzing indications and side-effects of different therapeutic modalities with illustrations and diagrams utilized throughout the book to present the information in a clear and schematic way. In this fully revised second edition, more than 120 new Q&As have been added and the answers to more than 90 Q&As has been modified after having critically incorporated all new knowledge generated in the past three years in the field of rheumatology, in an effort to bridge classical and current evidence-based knowledge and to present didactic and credible information. This book is valuable to test and acquire knowledge not only for rheumatologists but for every specialist in internal medicine, family practice, physical/rehabilitation medicine and orthopedic surgery.

The ability to intervene at the earliest phases in the pathogenesis of a chronic rheumatic disease caused by auto-inflammatory, autoimmune, or tissue injury mechanisms has th epotential to prevent disease manifestations and consequences, limited loss of quality of life, comorbidity, and costs to society. This issue is dedicated to exploring the stages of rheumatic disease, biologic mechanisms contributing to the pathogenesis along with possible ways to study and screen for persons at risk with the ultimate goal of finding ways to prevent these devastating diseases.

Scleroderma, or systemic sclerosis, is a chronic connective tissue disease generally classified as one of the autoimmune rheumatic diseases. Hardening of the skin is one of the most visible manifestations of the disease. It’s estimated that about 300,000 Americans have scleroderma. About one third of those people have the systemic form of scleroderma. Since scleroderma presents with symptoms similar to other autoimmune diseases, diagnosis is difficult. This issue will cover: The Genetic Basis of SSc: Genetics, Epigenetics, Mechanisms of Pathogenesis – linking fibrosis, vasculopathy and immune dysregulation, The Role of Autoantibodies in Diagnosis and Prognosis/survival; Managing Raynaud’s phenomenon and ischemic ulcers, managing SSc Lung Disease, Monitoring for and Managing Pulmonary Arterial Hypertension, and many more!

Recent developments in basic science and clinical rheumatology make it appropriate at this time to create a volume devoted to the immunology of rheumatic diseases. The impact of molecular biology, gene cloning, and new technologies for establishing hybridomas and T-cell lines in the laboratory is now beginning to be felt in clinical medicine. There is a general air of excitement and a feeling that we stand on the threshold of a new era in molecular medicine and clinical science. It is this excitement that we have tried to capture in this book. This volume is divided into five sections entitled Basic Mechanisms, Autoimmunity, Classical Concepts of Rheumatic Diseases, Pathogenetic Mechanisms, and Therapy. This is not an arbitrary arrangement but represents our belief that from an understanding of basic mechanisms of disease pathogenesis will come new and more successful forms of treatment for the sufferers of rheumatic disorders. We have tried in the selection of authors to choose internationally recognized experts who have both a scientific and a clinical orientation to their subjects. We believe the marriage of clinical and basic disciplines represents the best hope for rapid knowledge transfer from the laboratory to the clinic, where such knowledge can be used to improve patient health.
