Autonomic dysfunction is a major and poorly understood consequence of spinal cord injury. It is a cause of very serious disability and requires much more research. It should be a focus of treatment strategies. This book will be of interest to anyone involved in research and treatment of spinal cord injury since it helps to explain the tremendously negative impact on the body caused by cord injury that is not as obvious as paralysis and loss of sensation. It contains a compilation of what is known about bladder, cardiovascular, bowel and sexual dysfunction after spinal cord injury, as it relates to the changes within the autonomic nervous system control of these functions. The book begins with a description of the time course of autonomic dysfunctions and their ramifications from the first hours after a spinal cord injury to the more stable chronic states. The next section contains three chapters that address anatomical findings that may provide some of the foundation for autonomic dysfunctions in many of the systems. The system-specific chapters then follow in four sections. Each section begins with a chapter or two defining the clinical problems experienced by people with cord injury. The following chapters present research, basic and clinical, that address the autonomic dysfunctions.

The latter gives rise to respiratory and cardiac problems, temperature regulation disorders, endocrine system disorders, and many associated metabolic disorders. Other potential consequences of spinal cord injuries include pressure injuries and various disabilities and obstacles, ranging from physical limitations to social embarrassment. This handbook offers extensive guidance on medical management in different scenarios from the acute phase to long-term care, with a particular focus on information of importance for the solution of clinical problems commonly encountered in daily practice. It will be ideal for practitioners in rehabilitation medicine, neurosurgery, orthopedics, neurology, and other relevant specialties that deal with patients with spinal cord injuries.

Clinical Management Of Shock written by Stanislaw P. Stawicki and has been published by BoD – Books on Demand this book supported file pdf, txt, epub, kindle and other format this book has been release on 2020-06-24 with Medical categories.

Shock is a physiological state of war! From a healthcare provider perspective, the word “shock” is associated with a mixed array of feelings, including dread, well-founded fear, and deep respect. The physiological state of shock is well recognized for the associated destructive consequences, and its successful management requires prompt identification, immediate action, and sustained effort by all members of the healthcare team. This mindset of advanced preparation and constant readiness constitutes the foundation of the modern approach toward shock – early detection and prompt treatment for optimal outcomes. Despite the heterogeneity of “shock” as a clinico-pathological entity, there are some common threads that permeate all forms and manifestations of shock, with apparent increase in observed commonalities in the more advanced (and often irreversible) stages of the systemic syndrome. When faced with shock, the body and its systems do their best to compensate for the maldistribution of oxygen and nutrients. This is known as the so-called compensated shock. Beyond that, the body loses its ability to adjust any further, thus descending into “uncompensated shock,” with a refractory state characterized by vasoplegia and irreversible cardiovascular failure. As the reader journeys through the chapters of the book, he or she will read about various biomarkers and endpoints of resuscitation, explore different types of shock (e.g., septic, hemorrhagic, anaphylactic) and learn about some of the less often discussed topics such as neurogenic and spinal shock, as well as the amniotic fluid embolism. Our goals were to keep things clinically relevant and practically oriented, thus enabling the reader to apply the newly acquired knowledge in their everyday clinical routines. As the reader progresses through the book, we hope to help stimulate further discourse and innovative thinking about the topic. In this context, it is critical that basic, translational, and clinical research on shock continues to advance. Only through ongoing scientific progress can we help improve outcomes for patients with both rare and common forms of shock.

This fourth edition of Autonomic Failure (now available in paperback) covers the many recent advances made in our understanding of the autonomic nervous system. There are 20 new chapters and extensive revisions of all other contributions. Autonomic failure, fourth edition makes diagnosis increasingly precise by fully evaluating the underlying anatomical and functional deficits, thereby allowing more effective treatment. This new edition continues to provide practitioners from a variety of fields, including neurology, cardiology, geriatric medicine, diabetology, and internal medicine, with a rational guide to aid in the recognition and management of autonomic disorders. The book starts with an updated classification of autonomic disorders and a history of the autonomic nervous system. The first two sections of the book deal with the fundamental aspects of autonomic structure, function, and integration. There are new chapters dealing with neurobiology, nerve growth factors, genetic mutations, neural and hormonal control of the cerebral circulation, innervation of the lung, and pathophysiological mechanisms causing nausea and vomiting. Advances in the clinical management of autonomic disorders are critically dependent on the bridge made between the basic and applied sciences.
