This book presents a circumspective overview and update of the present existing knowledge of the biology, chemistry and pathophysiology of the dental pulp. It details numerous observations of a group of highly specialized investigators who have united in the common purpose of presenting their observations for the benefit of clinicians, teachers, researchers and students. Fortunately, the dental literature presents abundant research findings about pulp biology and the pulp's responses to various stimuli. This abundance has resulted in an increased interest and expansion of research on this subject. For example, publications abound on the response of pulp tissue to various medications and to a variety of types of dental materials which may be placed near to or at some distance from the pulp through the medium of dentine. One of the reasons the pulp is of such interest is that it not only provides the vitality to the teeth but also produces the dentine - both the primary and secondary, as well as reparative. The latter-type dentine is a result of the pulp's functions in response to disease as the former dentine is in response to health. As an example, some investigators have reported the effects of cutting of dentine and the placement of restorations in dentine which in turn reflect changes on the pulp tissue. These reports have raised a number of questions, which in turn have created a need for answers.

This presentation describes various aspects of the regulation of tissue oxygenation, including the roles of the circulatory system, respiratory system, and blood, the carrier of oxygen within these components of the cardiorespiratory system. The respiratory system takes oxygen from the atmosphere and transports it by diffusion from the air in the alveoli to the blood flowing through the pulmonary capillaries. The cardiovascular system then moves the oxygenated blood from the heart to the microcirculation of the various organs by convection, where oxygen is released from hemoglobin in the red blood cells and moves to the parenchymal cells of each tissue by diffusion. Oxygen that has diffused into cells is then utilized in the mitochondria to produce adenosine triphosphate (ATP), the energy currency of all cells. The mitochondria are able to produce ATP until the oxygen tension or PO2 on the cell surface falls to a critical level of about 4–5 mm Hg. Thus, in order to meet the energetic needs of cells, it is important to maintain a continuous supply of oxygen to the mitochondria at or above the critical PO2 . In order to accomplish this desired outcome, the cardiorespiratory system, including the blood, must be capable of regulation to ensure survival of all tissues under a wide range of circumstances. The purpose of this presentation is to provide basic information about the operation and regulation of the cardiovascular and respiratory systems, as well as the properties of the blood and parenchymal cells, so that a fundamental understanding of the regulation of tissue oxygenation is achieved.

The recogmuon recogmtIon of the microcirculation as an ideal interdisciplinary meeting place for the life sciences is really a postwar phenomenon. The European and the American Societies more than any other organizations launched the idea, and the success of the European Society's International Meetings gave impetus to a growth of interest from a handful of specialists to the wide interdisciplinary study which microcirculation now represents. The meeting held in Canada in June 1975 was, however, the first truly international meeting devoted to the microcirculation. It, too, was a success from every point of view, and the exchange of knowledge and new ideas was rewarding. It is our present hope that the tradition of European meetings with their characteristic European flavor will continue, but larded by larger, international congresses conceived on a worldwide basis. For the present conference we were fortunate in the presence of Dr. B. Zweifach. He was once referred to as the "father of the microcircula tion." This claim, unfortunately, I cannot accept. That honor probably belongs to Harvey, who by one of the most brilliant strokes of inductive reasoning in medical history inferred the existence of capillaries though he could not see them. Ben Zweifach's role was rather that of the midwife, presiding at the birth rather than the conception. The baby he delivered long years ago has since thriven lustily and its growth is in no small measure due to the continuing zeal of Zweifach and his associates.

The contact between the human blood and foreign technical apparatus (circulatory) and respiratory assist devices, arti ficial kidneys) leads to changes in the plasmatic and cel lular components of the blood, which have often only been taken as signs of gross but passive blood destruction. There is much evidence to support this notion in parts (mechanical cellolysis of erythrocytes and thrombocytes), however, these changes have to be seen in a broader bio logical context as an organ-specific reaction of the blood. It is an unjustified oversimplification to simply treat the blood as transport organ for respiratory gases (02 and CO ) and metabolites. The blood is rather also an organ 2 system for specific and unspecific defense mechanisms, di rected against the hazards of mechanical, microbiological and toxicological risks posed to multicellular macro-orga nisms in an environment in which trauma and other distur bances of the physical integrity are common. The two defense systems of the blood are based on humural and cellular constituents; the latter have to be considered as "excitable cells", capable of responding in a predictable and automatic fashion to adequate stimuli. The response of the cellular constituents in controlled and coordinated by chemical mediators. The immunological research of the last century has distinguished two separate defense systems of the blood: I. The highly specific defense system (immune system sen sustrictori) which is phylogenetically new and is ontogenetically learned.
