The annual meeting of the Perinatal Biochemical Group of the Spanish Biochemical Society held in Madrid on December 15-16, 1989, provided an excellent opportunity to bring together a group of distinguished investigators both from Spain and from abroad with a common interest in developmental endocrinology and biochemistry. The aim of the symposium was to present and discuss the most recent developments in the areas of endocrine and biochemical processes critical to normal growth and maturation of the newborn. To achieve a high degree of interaction among the participants, subject reviews as well as short communications were include in the program. The reviews provided in-depth information on selected important topics. The purpose of short communications was two fold: (i) to provide a forum to discuss on-going investigations on related areas; and (ii) to present opportunities for active participation by young investigators. This format proved very successful in generating fruitful discussions among the participants. Taken together the review chapters and the short communications have resulted in a coherent and unified subject presentation. Advances in biochemistry, molecular biology and cell biology have provided not only new and exciting experimental approaches but also have opened up new directions in the investigation of differentiation and developmental processes at cellular, molecular and biochemical levels during the early stages of growth and maturation. In recent years a wealth of information in these areas of development has emerged at a rapid rate.

Survival of extremely premature neonates has improved significantly following the advances in neonatal intensive care. Extrauterine growth restriction is a serious issue in this population. Nutritional exposures during critical period of life influence the individual’s risk of disease throughout life. Nutritional deficit and poor growth are associated with long term neurodevelopmental impairment, short stature and metabolic disorders in extremely preterm neonates. Optimising nutrition in the early postnatal life of the preterm neonate is therefore a priority. However this is easier said than done considering the frequency of feed intolerance, fear of necrotising enterocolitis, and the hesitancy in adopting an aggressive approach to parenteral nutrition in this population. Some of the finest researchers in the field have come together to provide the clinical perspective on the A to Z of nutrition in the preterm neonate in simple and clear fashion in this book.

During the mid- to late-twentieth century, study of the physiology of the developing fetus and newborn infant evolved rapidly to become a major discipline in the biomedical sciences. Initially of interest from a standpoint of function of the placenta and oxygenation of the fetus, the field advanced to explore both normal functional mechanisms as well as pathophysiologic aspects of their regulation. Examples include studying the role and regulation of circulatory vascular anatomic shunts in oxygenation, cardiac function, certain aspects of asphyxia in the fetus and newborn infant, the role of fetal “breathing” movements, cyclic electroencephalographic activity, and analysis of electronic monitoring of fetal heart rate variability and its significance. Included in this book are reminisces of several dozen individuals who played a vital role in these developments. Overall, this survey considers a number of aspects of the development of the science of fetal and neonatal physiology, and its role in the greatly improved care of pregnant women and their newborn infants. The account of "neonatal sterilization" is the story of the advocates of direct effect of steroids on the gonads and those who believed in the indirect influence, mediated through the hypothalamus and/or the pituitary gland. As often happens in biology, both convictions represent the same image seen from different perspectives. Prof DC Johnson (Kansas City, KS) reminisced the beginning of the story in a letter to me. I am paraphrasing parts of the letter with his permission. "As a starting point we could pick the life-long research of Emil Steinach ... " Steinach recognized the influence of testes on the develop ment of accessory sex organs in 1894, described virilization of females and feminization of males in 1913, and identified the controlling influence of the hypophysis on the gonads in 1928. He reviewed his work in a book Sex and Life, Forty Years of Biological and Medical Experience (E Steinach and L Loebe!; Faber and Faber, London, 1940). He got on the wrong road in later years and that is the reason everybody seems to have forgotten him. He presented his hypothesis that estrogen has a direct effect upon the testes, i. e. hormone antagonism, at the 1st International Congress on Sex Research in 1926.
