This book traces the assemblage that comes into being in the spaces and experiences of childbirth. Charting the contributions of the multiple human and non-human actors that contribute to the birth experience, it offers a new perspective on childbirth that cuts across the often emotional debates about natural versus medicalised birth. Scholars turn to reproduction for its ability to illuminate the practices involved with negotiating personhood for the unborn, the newborn, and the already-existing family members, community members, and the nation. The scholarship in this volume draws attention to doula work as intimate and relational while highlighting the way boundaries are created, maintained, challenged, and transformed. Intimate labour as a theoretical construct provides a way to think about the kind of care doulas offer women across the reproductive spectrum. Doulas negotiate boundaries and often blur the divisions between communities and across public and private spheres in their practice of intimate labour. This book weaves together three main threads: doulas and mothers, doulas and their community, and finally, doulas and institutions. The lived experience of doulas illustrates the interlacing relationships among all three of these threads. The essays in this collection offer a unique perspective on doulas by bringing together voices that represent the full spectrum of doula work, including the viewpoints of birth, postpartum, abortion, community based, adoption, prison, and radical doulas. We privilege this broad representation of doula experiences to emphasize the importance of a multi-vocal framing of the doula experience. As doulas move between worlds and learn to live in liminal spaces, they occupy space that allows them to generate new cultural narratives about birthing bodies.

When House Speaker Paul Ryan urged U.S. women to have more children, and Ross Douthat requested “More babies, please,” in a New York Times column, they openly expressed what policymakers have been discussing for decades with greater discretion. Using technical language like “age structure,” “dependency ratio,” and “entitlement crisis,” establishment think tanks are raising the alarm: if U.S. women don’t get busy having more children, we’ll face an aging workforce, slack consumer demand, and a stagnant economy. Feminists generally believe that a prudish religious bloc is responsible for the protracted fight over reproductive freedom in the U.S. and that politicians only attack abortion and birth control to appeal to those “values voters.” But hidden behind this conventional explanation is a dramatic fight over women’s reproductive labor. On one side, elite policymakers want an expanding workforce reared with a minimum of employer spending and a maximum of unpaid women’s work. On the other side, women are refusing to produce children at levels desired by economic planners. By some measures our birth rate is the lowest it has ever been. With little access to childcare, family leave, health care, and with insufficient male participation, U.S. women are conducting a spontaneous birth strike. In other countries, panic over low birth rates has led governments to underwrite childbearing and childrearing with generous universal programs, but in the U.S., women have not yet realized the potential of our bargaining position. When we do, it will lead to new strategies for winning full access to abortion and birth control, and for improving the difficult working conditions U.S. parents now face when raising children.

Most women give birth in hospitals, institutions modelled around the needs of the people who work there. The delivery room is designed around the obstetric bed which was designed for the benefit of the obstetrician rather than the woman giving birth. Despite research showing the benefit of upright positions in labour and birth, most women in the UK still give birth in the semi-reclined position, pushing their baby out against the forces of gravity. The author argues that unnatural positions make labour and birth more painful and difficult for modern women than it was for their ancestors. How did we come to put the needs of care givers above those of the labouring woman? Is there anything that can be done? Starting with a short history of birth furniture, Dynamic Positions in Birth goes on to explore the anatomy and physiology of labour from an evolutionary perspective and explores how rethinking positions for labour and birth could benefit mothers and their babies. Equally important is the need to change attitudes to birth so that women are encouraged to play a more active part in the birth of their babies instead of being subjected to clinical interventions designed to mitigate the adverse effects of labouring in a starkly unnatural environment. Margaret Jowitt argues that it is possible to give women labouring in hospital a better chance of giving birth naturally. The book concludes by calling for a fresh look at the environment for birth. Delivery rooms can be made more user friendly by introducing furniture designed around women’s need for physical support during labour as well as for the birth, and by hiding away the more alarming technology unless it is needed. Women need a less forbidding environment and more encouragement to move freely and adopt positions which will enhance their chance of achieving a normal birth.

The need for nonmaternal child care has risen dramatically as increasing numbers of mothers with preschool children have entered the labor force. This report considers the effects of child care costs on the supply of new mothers in the work force. The authors examine the role of federal and state subsidies--in the form of income tax deductions and credits for child expenses--in the rapid increase in work among mothers of very young children. They focus on the labor supply of women immediately following their first birth in order to understand to what extent child care subsidies have promoted the rapid growth in labor supply among mothers of very young children. Finally, they discuss a model of women's return to work and their implementation of that model using data from the National Longitudinal Survey of Youth. They also include empirical estimates of the effect of child care costs on return to work after childbirth.
