Editors Ayşe Serap Karadağ, Ümit Tursen Series editor Torello Lotti Translation Editor Linda Gray Abstract Botulinum neurotoxin (BoNT) is a toxin that is secreted by Clostridium botulinum bacteria. BoNT is used for many cosmetic and non-cosmetic dermatological indications. Its FDA- approved indications are limited to, for example, axillary hyperhidrosis, glabellar striae, crow's feet, and the forehead region. BoNT was first approved by the FDA for use in dermatology to treat moderate to severe glabellar lines in 2002. Later on, new indications continued to be added. It has been employed in many diseases in recent years, and has been used successfully, often in advance of FDA approval. Among these indications are sweat gland disorders, pruritic dermatoses, notalgia paraesthetica, Raynaud phenomenon. Fox–Fordyce disease, Hailey-Hailey disease, hidradenitis suppurativa, keloid and hypertrophic scars, alopecia, facial erythema and flushing, and psoriasis. The greatest advantage of BoNT therapy is that its systemic side effects are rare and usually temporary. However, its main disadvantage is the need for the frequent repetition of injections because of its short duration of action (3-4 months). Factors such as injection technique, dilution, storage, and frequency of application affect the success of treatment. In this book, the indications for the use of botulinum toxin for non-cosmetic and therapeutic purposes in dermatology are explained, and practical information that can be quickly reviewed is presented. The tables in the chapters provide short and concise information for those who want to summarize the topics quickly. We hope it will be a useful work that you will use in your daily practice. This book, containing more than 400 photos and illustrations, provides practical guidelines for the cosmetic use of botulinum toxin type A (BTA) in Asians. The differences in BTA treatment of Asians and Caucasians with respect to applicable dose, injection methods, anatomic significance, and indications are clearly described. It is explained how the optimal dose and injection sites for the treatment of wrinkles differ from the guidelines advocated in North America and Europe. Detailed consideration is given to the rapidly expanding role that BTA treatment is playing in facial and body contouring based on leveraging the mechanism of disuse muscle atrophy. Examples that are particularly relevant in Asians include treatment of hypertrophy of the masseter and temporalis muscles and calf muscle reduction. Further chapters are devoted to the use of BTA in the treatment of hyperhidrosis and the intradermal injection of BTA. The book will be an excellent resource for all dermatologists, plastic surgeons, cosmetic Physicians, and other clinicians who employ BTA in Asian patients.

Even those clinicians who feel themselves far from aesthetic dermatology now recognise that BoNT-A is an extraordinary tool in their therapeutic armamentarium for treating complex disorders such as focal idiopathic hyperhidrosis. This has opened a therapeutic scenario for a group of patients very difficult to treat previously. Since the introduction of BoNT-A in the dermatological therapeutic armamentarium, in 1990, things have changed to the point that any dermatologist should now have knowledge of BoNT-A basics, and the practicalities of BoNT-A vial reconstitution and injection. Moreover, in the last years, a widespread availability of multiple different commercially available forms of BoNT-A has been for sale, thus the clinicians must now really know and consider the advantages and disadvantages of the different available products. These would include dosing/duration, immunogenicity, and spread of effect. What has not changed over the years is the safety profile of BoNT-A. The knowledge regarding its side effects and complications have greatly increased, confirming that, despite the attempts of some individuals to assert otherwise, BoNT-A remains one of the safest forms of treatment available to us as physicians. This edited collection provides a comprehensive practical guide on the use of botulinum toxin in several dermatological disorders, including focal idiopathic, axillary, palmar and plantar, craniofacial or segmental hyperhidrosis, chrono-aging, hidradenitis, and gives some suggestion on the use of BoNT-A for the treatment of several orphan skin diseases, not responding to conventional treatments like benign Hailey-Hiley disease, lichen simplex, osmidrosis. Using clear explanations, simple line drawings, diagrams and clinical pictures, it provides suggestions to improve practical knowledge on the use of BTX-A in dermatology. Hot topics include practical information for clinicians regarding the correct counselling of the patients to be treated; methods and ready to use tests to select patients to be treated; the optimal dosage and dilution according to the site of injection and the indication of use; the relevant injection sites for each condition; the correct technique of injection in the different areas of body surface; comparative dosage tables for the various formulations of toxin used; how to manage and prevent side effects related to the use of BoNT-A; common pitfalls and how to optimize outcomes; timing for the follow-up of treated patients. The authors hope that this book may be a starting point for the use of a drug that is still growing, and a useful tool for those clinicians who want to improve their knowledge about the use of BoNT-A in dermatology.
