The present volume is based upon the invited review lectures delivered to the European Brain and Behaviour Society's Workshop on Recovery of Function Following Brain Damage held at Goldsmiths' College, University of London, in April 1991. Coming exactly ten years after the Society's ftrst meeting on this subject, held at Erasmus University, Rotterdam, a major objective of the Workshop was to review progress in the intervening years. This task was begun by Professor D. G. Stein in his opening presentation. Looking ahead to possible developments in recovery research in the next decade was the subject of Professor B. Kolb's closing lecture. The intervening presentations reviewed progress made in speciftc areas of recovery research. In addition to reviewing progress over the last decade we sought to achieve an additional objective in the way that the invited review lectures were organised. This was to bring together those doing basic research, usually animal research, and those whose of the lectures were "paired", research interests are more clinically orientated. Thus some one concentrating on the results of animal studies and one on clinical research findings.

"Brain Damage - Bridging Between Basic Research and Clinics" represents a collection of papers in an attempt to provide an up-to-date approach to the fascinating topic of brain damage in different pathological situations, combining the authors' personal experiences with current knowledge in this field. The idea for the present volume grew from discussions that the four of us had among ourselves and with our colleagues at recent scientific meetings. All of us were impressed by the wealth of empirical data that was being generated by investigators interested in brain damage and recovery from both behavioral and biological orientations. Nevertheless, we were concerned about the relative paucity of attempts to evaluate the data provided by new technologies in more than a narrow context or to present new theories or reexamine time-honored ideas in the light of new findings. We recognized that science is guided by new technologies, by hard data, and by theories and ideas. Yet we were forced to conclude that, although investi gators were often anxious to publicize new methods and empirical fmdings, the same could not be said about broad hypotheses, underlying concepts, or in ferences and speculations that extended beyond the empirical data. Not only were many scientists not formally discussing the broad implications of their data, but, when stimulating ideas were presented, they were more likely to be heard in the halls or over a meal than in organized sessions at scientific meetings.

A distinction between primary and secondary brain damage of vari ous origin, particularly in acute lesions, such as head injury and ische mia is not entirely new. The concept is of practical significance, be cause it is the foremost intention of all clinical efforts to prevent, or at least attenuate the development of secondary sequelae. Primary dam age to nervous elements usually cannot be influenced by treatment. Its prevention is the objective of prophylactic measures. The current volume gathered prominent scientists and clinicians from various fields to pro vide a competent introduction and survey of the various aspects involved in secondary brain damage. It was attempted to provide criteria for the distinction between the primary and secondary phenomena on a morpho logical and functional level, on the basis of the kinetics involved and, most importantly, regarding the different specific manifestations, such as disturbances of microcirculation, aspects of the blood-brain barrier, and of cellular structure and function at a molecular level. Although it was not expected that a grand unifying hypothesis will be reached recon cilable with the many, occasionally opposing views on such a complex subject, nevertheless, the present volume attains an appropriate result. It can best be described as a mosaic of many different pieces which only as an ensemble reflect the current state of the art.

Basic Issues in Rehabilitation of the Brain Damaged Definitions Because of the vagueness surrounding the term brain damage, it is nec essary at the outset to define the population to which this book may have some application. Although it is usual to speak of the brain damaged patient in a general way, the conditions referred to cover a variety of specific disorders. In this book we will be discussing only individuals who become brain-damaged as adults. We will be ad dressing ourselves specifically to adults who have sustained demon strable, structural brain damage. Those conditions in which brain dys function is a possible etiological agent, such as a number of functional psychiatric disorders, will not be considered. Thus the entire topic of mental retardation and early life brain damage will not be treated here, nor the many problems associated with minimal brain damage syn dromes in school age children. Modern psychiatric thinking has tended to blur the distinction between the so-called functional and organic disorders (d. Shagass, Gershon, & Friedhoff, 1977), but we would ad here to the view that the patient with structural brain damage contin ues to present relatively unique assessment and treatment problems. Furthermore, the emphasis of this book will be placed on individ uals with nonprogressive, chronic brain damage.

The publication of the Vth International Symposium 1995 on "Mechanisms of Secondary Brain Damage" in Mauls/ltaly is a collection of focused reviews reaching from novel molecular- and cell biological findings to aspects of clinical management in head injury and cerebral ischemia. A specific purpose of these series of meetings introduced in 1984 is for an exchange on problems of mutual interest by international high ranking experts from the basic sciences and related clinical disciplines, such as intensive care medicine, neurology, or neurosurgery. The present volume covers three major areas: (a) Molecular and cell biological mechanisms including inflammation (b) Novel findings on mechanisms and treatment in cerebral ischemia (c) Secondary processes in head injury, regeneration and treatment Molecular-and cell biology is currently attracting attention towards activation of genomic processes associated with the demise of cells referred to as "programmed cell death" and "apoptosis" which, actually, might be distinguished from each other. Thus, the phenomenon of delayed neuronal death in selectively vulnerable brain areas following brief interruption of blood flow is scrutinized as to the contribution of the activation of suicide genes. The physiological role of such a response, among others, is removal of surplus neurons during ontogenesis of the brain. Yet, evidence is accumulating that similar mechanisms playa role in cerebral ischemia, probably also trauma, where nerve-and other cells demonstrate features of apoptosis. Observations on protection of neurons by administration of protein synthesis inhibitors in cerebral ischemia provide more direct support.
