This volume contains the papers presented at the 24th Annual Meeting of the Deutsche Gesellschaft fur Neurochirurgie, held in Mainz, Western Germany, on April 30 - May 3, 1973. Deliberate choice was made of two crucial still hotly debated subjects which, for ages, have meant a source of constant worry, and nights without sleep to every neurosurgeon. Just as long as our special field exists, there have been the problems of how to control brain edema and ·of how to reduce lethality and the secondary lesions in surgery of cerebello-pontine angle tumors. Concerning the first subject, new pathological, pathophysiological and chemical aspects, the mechanisms of brain edema formation and resolution are presented in the hope for better understanding. Furthermore, the relationship between brain edema, intracranial pressure, cerebral blood flow and metabolism are discussed. Finally, the therapeutical consequences as well as the results of experimental and clinical work are presented, and a comparison of effects between different methods (hypertonic solutions, diuretics, steroids, controlled hyperventilation, hyperbaric oxygen) is given. Concerning the second main subject, any important contributions to the early diagnosis of cerebello-pontine angle tumors have been included. Nevertheless, it is of utmost interest for the neurosurgeon to know which approach he is to prefer for the different stages of tumor size and to be familiar with the trans labyrinthine approach or the posterior craniotomy, as well as with the importance of the use of the microscope in neurosurgery, the preservation of the facial nerve and, in certain cases, its repair.

This Atlas is one of a series devoted to neurosurgical and neuro logical conditions and is complementary to Atlas of the Histology of Brain Tumors (Springer-Verlag, Berlin-Heidelberg-New York 1971), which was the first in the atlas series. The Atlas is based on the Handbuch der Neurochirurgie, Vols. I and III (Springer 1956, 1959) but, whereas this is a comprehensive reference work, the present book is intended to give the practicing neurosurgeon, neuroradiolo gist, neuropathologist and neurologist the concise information they need for diagnostic purposes concerning the aspect, site, and ma lignancy of tumors and other space-occupying lesions in the brain. The schematic diagrams showing the sites of predilection of these tumors, as well as a piOgnosis based on the degree of malignancy, will be most useful here. The early chapters discuss the general rules governing displace ments due to space-occupying lesions and the manifestations of brain herniations. Other neurosurgical conditions, such as localized inflammatory processes, edema and obstructive hydrocephalus, are dealt with in brief chaptets; in this case I have chosen to show some of the rarer conditions rather than all the common lesions. In spite of probable future changes in terminology and classification, we have retained the classification used in the Atlas of Histology of Brain Tumors.

The current book represents a distillation of the experience gained in diagnosis of intracranial tumors with computed X-ray tomography at the University Hos pitals of Berlin, Mainz, and Miinchen. To what purpose? Standard radiological techniques such as pneumoencephalography with lumbar puncture and cerebral arteriography with puncture of the common carotid artery are invasive proce dures which entail a certain amount of risk as well as discomfort for the patient. Furthermore, diagnoses made with these procedures rely primarily on indirect signs of an intracranial space-occupying lesion - such as displacement of the air-filled ventricles or of normal cerebral vessels. Only a few types of tumor are demonstrated directly with these techniques. In contrast, computed tomography demonstrates the pathology directly in almost all cases, and this with a minimum of risk and discomfort. In addition, normal intracranial structures are demonstrated, so that the tumor's effect on its surroundings can be evaluated. Today, almost a decade after HOUNSFIELD'S revolutionary invention, diagno sis of brain tumors without computed tomography is almost unthinkable, if not in fact irresponsible.

This book represents the second, fully revised edition of the original volume published in 1982. Experience in neuroradiology has confirmed the outstanding value of computed tomography (CT) for the diagnosis of space-occupying lesions within the skull and orbit. It might be assumed, then, that the second edition of this book would simply represent a numerically expanded continua tion of the popular first edition. That is not the case, however. Advances in imaging techniques have promp ted the creation of a new book whose expanded title reflects its more comprehen sive nature. The added illustrations, the revised text, and the expanded circle of editors and contributors document this. Since publication of the first edition, a new modality, magnetic resonance imaging (MRI), has become an established neuroradiologic study. We felt it was essential to include this new modality in our book and explore its capabilities as an adjunct or alternative to CT scanning. Because of the high acquisition costs of MRI and the still small number of MR units currently in operation, we have relied in part on images furnished by other institutions and private practitioners, to whom we are indebted. Many problems relating to MR, both in terms of equipment and image interpretation, have yet to be resolved. There is no denying that we still have much to learn.
