Brucellosis, also known as undulant fever, Mediterranean fever, or Malta fever, is an important human disease in many parts of the world. Brucellosis is a major zoonotic disease that may cause a serious illness in humans and animals. Global prevalence of human brucellosis remains significant. More than half a million new brucellosis cases from 100 countries are reported annually to the World Health Organization (WHO). The majority of these cases are reported in developing countries.In humans, brucellosis (undulant fever, Malta fever) is characterized by an acute bacteremic phase followed by a chronic stage that may extend over many years and may involve many tissues. It is a systemic disease, and many organ systems (nervous system, heart, skeletal system, bone marrow, etc.) may become involved following hematogenous dissemination. Although eradicated in some countries, it remains one of the most economically important zoonosis worldwide as it is responsible for huge economic losses as well as significant human morbidity in endemic areas. Because of the nonspecific clinical manifestations of human brucellosis and the need for prolonged combination therapy with antibiotics that are not routinely prescribed for other infectious diseases, laboratory confirmation of the diagnosis is of paramount importance for adequate patient management. In addition, evidence of brucellosis has serious public health implications because it discloses exposure to a contaminated source (infected animals or their products, unsafe laboratory practices, or a potential biological warfare attack). This book addresses human brucellosis with stress on symptoms including those related to the less recognized disease localizations, risk of exposure, treatment, and prevention. Light is shed on animal brucellosis as it pertains to human exposure. The book also emphasizes on laboratory procedures in culturing and serologic techniques. Epidemiologic surveillance is among this books subjects as well as veterinary control measures.

Brucellosis is still endemic in countries of the former Union of Soviet Socialist Republics (USSR), such as Kyrgyzstan. Today, despite extensive control measures, human and livestock seroprevalence is prominent in all administrative regions of Kyrgyzstan. The lack of epidemiological data from both, the public and veterinary sector, and the genetic characterization of Brucella isolates currently pose a major challenge to the implementation of a more effective action plan to prevent further spread of brucellosis infection in this country. The general objectives of this study were: 1) to investigate the epidemiology of brucellosis in humans and animals; 2) to investigate the genetic diversity of Brucella species in human isolates, and their phylogenetic placement relative to the global diversity. Thus, a whole genome analysis of Brucella strains isolated from patients in 2019-2020, a systematic study on epidemiological data in humans and animals, and a review of the animal production, effectiveness of vaccines, transhumance movement of livestock covering the last 10 years have been done. Epidemiological data on brucellosis cases from the Republican Center for Quarantine and Highly Dangerous Infections Ministry of Health (RCQHDI) and data on livestock from the State Inspectorate for Veterinary and Phytosanitary Safety of the Kyrgyz Republic (SVPS) from 2010 to 2020 were analyzed to describe trends of the recent past and to draw an accurate picture on demographic context of the disease, involvement of cattle and small ruminants in humans disease and exchange of agents of Kyrgyzstan between different regions. All strains were collected as part of the standard clinical investigation of patients with suspected brucellosis. To the best of my knowledge, this is the first study on the molecular epidemiology of brucellosis in humans for Kyrgyzstan at all.

It is tobe hoped that, at a later date, when more comprehensive data become available, the Heidelberg Academy will be able to decide to support a cartographic presentation of the occurrence of brucellosis in these parts of the world as weil. Last, but not least, the intensive research carried out by bacteriologists had led to the discovery of new species of Brucella, such as B. ovis, B. canis and B. neotomae. Further types - such as Type 4 of B. suis which occurs in reindeer and caribou - were identified. Though these new species and types, which are undoubtedly to be classed with the brucellae, have achieved regional significance in places, the three dassie species B. melitensis, B. abortus and B.
