Over the past decade, there have been many international calls to strengthen and support/sustain research capacity in lower- and middle-income countries (LMICs). This capacity is considered an essential foundation for cost-effective healthcare systems. While there have been long-standing investments by many countries and research funding organisations in the training of individuals for this purpose, in many LMICs research capacity remains fragmented, uneven and fragile. There is growing recognition that a more systems-oriented approach to research capacity-building is required. Nonetheless, there are considerable gaps in the evidence for approaches to capacity-building that are effective and sustainable. This book addresses these gaps, capturing what was learned from teams working on The Global Health Research Initiative. This book brings together the experiences of research capacity-building teams co-led by Canadians and LMIC researchers in several regions of the world, including China, Chile, Jamaica, Kenya, Mexico, Nigeria, South Africa and Uganda.

Progress towards a world where all can enjoy optimal health and health care is crucially dependent on all kinds of research including research involving humans. Involving humans in medical research is necessary to improve the knowledge base on which medicine should be based. At the same time, individuals participating in health-related research have individual human rights and have a right to be protected against the risks that research may bring to them. The tension between these two considerations has led the medical community to endorse ethical guidelines for health-related research. Research Ethics Committees can use these guidelines to evaluate whether a given research protocol is ethically acceptable or not. CIOMS, in association with the World Health Organization, started its work on ethics in health-related research in the late 1970s. Accordingly, CIOMS set out, in cooperation with WHO, to prepare guidelines to indicate how the ethical principles set forth in the Declaration of Helsinki of the World Medical Association, could be effectively applied, particularly in low-resource settings, given their socio-economic circumstances, laws and regulations, and executive and administrative arrangements. Since then revised editions of the CIOMS ethical guidelines were published in 1993 and 2002. New developments in research have prompted CIOMS to again revise their ethical guidelines. The result is now available in this new publication. In the new 2016 version of the ethical Guidelines, CIOMS provides answers to a number of pressing issues in research ethics. The Council does so by stressing the need for research having scientific and social value, by providing special guidelines for health-related research in low-resource settings, by detailing the provisions for involving vulnerable groups in research and for describing under what conditions biological samples and health-related data can be used for research. In providing this new version, CIOMS hopes to ensure that the ethical Guidelines remain a living document that provides reasoned conditions for research in order to meet the challenges of modern research.

As health systems all over the world not only recover from COVID-19, but learn to adapt to contexts of increasing uncertainty amidst persistent challenges, it is clear that systems thinking has never been needed more. Systems thinking is an approach to problem-solving that views problems as part of a wider dynamic system. It recognizes and prioritizes the understanding of linkages, relationships, interactions and interdependencies among the components of a system that give rise to the system’s observed behaviour. Systems thinking is a philosophical frame, and it can also be considered a method with its own tools. Identifying ways in the short and long-term which strengthen health systems is critical and applied systems thinking offers opportunities to do this. Systems thinking is often considered to be a field, a discipline, a philosophical approach and a set of tools and methods and can be defined as a way to understand and improve complex issues and situations. Despite broad consensus that systems thinking is important in health systems strengthening, it remains underutilized by researchers, public health practitioners and health decision makers. Further, a gap remains in the translation from concept to policy.
