The 2018 FAO-OIE-WHO (Tripartite) zoonoses guide, “Taking A Multisectoral, One Health Approach: A Tripartite Guide to Addressing Zoonotic Diseases in Countries” (2018 TZG) is being jointly developed to provide member countries with practical guidance on OH approaches to build national mechanisms for multisectoral coordination, communication, and collaboration to address zoonotic disease threats at the animal-human-environment interface. The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

Antimicrobial resistance (AMR) occurs when microbes, bacteria, viruses, fungi, and parasites, evolve in ways that reduce medicine's ability to fight them. AMR has made many infections, particularly bacterial infections, increasingly difficult or even impossible to treat. Without effective medicines, the number of people with severe microbial infections will increase, as will the number of people who die from these infections. In 2019, 4.95 million deaths were associated with AMR, with the highest AMR-attributable death rates occurring in western sub-Saharan Africa.8 While AMR is a natural evolutionary process that occurs over time, many human-led behaviors have increased its occurrence. The main drivers of AMR include: (1) the misuse and overuse of antimicrobials; (2) lack of access to clean water and adequate sanitation for people and animals; (3) poor infection prevention and control measures in healthcare facilities and farms; (4) limited access to quality, affordable medicines, vaccines, and diagnostics; and (5) lack of awareness and knowledge about AMR. Additionally, for countries to progress from planning to acting, AMR coordination committees must have a clearly defined role and the remit to develop and implement an operational plan. Operational plans should embed NAP activities into the national development agenda, sectoral strategies, and budgets, and should coordinate an aligned approach to delivering NAP activities across sectors and stakeholders. Creating and empowering AMR coordination committees and comprehensively implementing and monitoring the prioritized activities outlined in their NAPs operational plan takes time; but there are several relatively easy entry points for AMR action that countries can utilize to accelerate their fight against AMR. The remaining case studies in this series showcase successful actions against AMR that three countries, Burkina Faso, Jordan, and Malawi, have taken along their journey to fully operationalizing and implementing their NAPs on AMR.

The societal consequences of antimicrobial resistance (AMR) are severe. They include declining health outcomes from longer illnesses, prolonged stays in hospital, loss of protection for patients undergoing medical procedures, increased health care expenditure, and increased mortality. They also include declining global food security as AMR damages farm animal health and crop yields. Despite AMR being a transboundary crisis, concerted global initiatives that effectively combat AMR have been few and far between. Steering Against Superbugs analyses ways to reduce barriers and create opportunities for coordination. Antimicrobial resistance (AMR) is the ability of microorganisms to fight antimicrobial compounds, reducing the efficacy of treating diseases in humans, animals, and plants. AMR risk is outpacing human population growth, owing to misuse of antimicrobials in large quantities in food systems, and is a serious threat to food security and sustainable development. FAO, with the World Health Organization (WHO), the World Organization for Animal Health (OIE), and the United Nations Environment Programme (UNEP), is supporting countries in developing and implementing their One Health National Action Plans on AMR. The eventual aim is to ensure sustainable use of antimicrobials to minimize AMR risks, in alignment with the Global Action Plan on AMR. The scope of the evaluation covers FAO’s entire work on AMR up to early 2020 and its role in the global AMR architecture. It examines FAO’s organizational and institutional set-up for AMR work. FAO has a strong mandate to work on AMR, implementing activities in 45 countries and providing far-reaching support on AMR National Action Plans (NAPs). FAO’s technical expertise is a key comparative advantage in its work on AMR. It is underpinned by the strong scientific grounding of FAO’s work, engendered in its AMR working groups and supported by its collaboration with research centers, universities, and the Tripartite organizations. Nevertheless, the work is relatively recent and, given the long impact pathways, it has had limited results. A comprehensive strategic and programmatic approach would increase the likelihood of achieving results in combating AMR. FAO should prioritize its work in a long-term strategy on AMR that recognizes the seriousness of the threat and is fully integrated into the Organization’s Strategic Framework. The strategy should set out FAO’s long-term role in combating AMR and that of its divisions and offices, as well as its approach at the country and regional level. FAO should consolidate its work on AMR through a strong programmatic approach with a central coordination and management structure that links with the Regional Offices and is supported by dedicated core funding.
