Background: Amselu2019s clinical criteria, defined by presence of >3 of vaginal pH 4.7, clue cells, positive whiff test, and homogenous discharge, are frequently used to identify women for enrollment in studies of BV. Vaginal dysbiosis, including u201cintermediate florau201d defined by Gram stain (Nugent score), is common among women in sub-Saharan Africa.Methods: We determined prevalence of individual Amselu2019s criteria among women screened for enrollment into a prospective cohort study that aimed to measure effects of a vaginal contraceptive ring (CVR; NuvaRing) on the vaginal microenvironment. Because limited data suggest that this CVR increases concentrations of Lactobacillus crispatus, we preferentially enrolled women with clinical diagnosis of BV; women with asymptomatic BV were eligible. Other inclusion criteria included age 18-40 years, capable of providing consent, and not wishing to become pregnant during participation. We assessed the extent to which women were excluded from participation based on presence of only 2 of 4 Amselu2019s criteria.Results: We screened 520 women, of whom 93 (18%) were HIV-infected. Nearly half (47%) had abnormal vaginal discharge consistent with BV on examination, over half (53%) had pH 4.7, and approximately one third each had positive whiff test (32%) or clue cells on saline microscopy (28%). Findings were similar for women with HIV. Of all screened, 160 (31%) had >3 Amsel criteria, and were eligible for enrollment. Of the remainder, 95 (18%) had two Amsel criteria, and 67 (13%) had one. Of those with only two criteria, pH was abnormal in 34%.Conclusions: Nearly one-third of women who were ineligible for enrollment were not eligible because they had only 2 of 4 Amsel criteria detected, yet of these, 1/3 had abnormal pH, suggesting a suboptimal vaginal environment. Excluding this large number on the basis of only 2 of 4 Amselu2019s criteria created inefficiency in study operations, slowed enrollment, and, given high prevalence of BV and dynamic vaginal microbiota, likely excluded many women who may have derived benefit from participation. More accurate methods of screening women for BV, including molecular diagnostic tests, should be considered in this setting.

Non-gonococcal urethritis (NGU) is the most common urethral syndrome in the United States, yet up to 50% of cases are of unknown etiology. Bacterial-vaginosis associated bacteria (BVAB), highly predictive of BV in cisgender women, have been detected in cisgender men and may explain some cases of NGU. We sought to determine the frequency and correlates of three BVAB in cisgender men and evaluate whether they were associated with NGU. Methods: We enrolled cisgender men who have sex with women (MSW) from a sexually transmitted disease (STD) clinic in Seattle, WA. Participants completed a computer-assisted self-interview (CASI), underwent a clinical interview and examination, and provided a urethral swab and urine specimen. We quantitated polymorphonuclear leukocytes (PMNs) on a Gram stained urethral smear and defined NGU as >̲5 PMNs per high power field plus either urethral symptoms or visible discharge in the absence of Neisseria gonorrhoeae. First void urine specimens were tested for Chlamydia trachomatis (CT) and Mycoplasma genitalium (MG), using Aptima assays (Hologic, Inc. San Diego, CA) and for BVAB2, Mageeibacillus indolicus, and Sneathia spp. using laboratory-developed quantitative polymerase chain reaction (qPCR) assays. We also tested a subset of men for Trichomonas vaginalis (TV), Ureaplasma urealyticum (UU), herpes simplex virus, and adenovirus using qPCR. We used logistic regression to evaluate potential associations between BVAB and NGU. Results: A total of 317 MSW met inclusion criteria and were included in these analyses. Of these, thirty-six (11.4%) had BVAB2, 17 (5.4%) had M. indolicus and 67 (21.1%) had Sneathia spp. No sociodemographic characteristics were associated with any of the BVAB. However, men with each of these three BVAB were significantly more likely to report >̲3 sex partners in the past two months (BVAB2: 47.2% vs. 23.0%, p=0.01; M. indolicus: 52.9% vs. 24.2%, p=0.03; Sneathia spp.: 42.2% vs. 21.5%, p
