Despite the advent of the genomic era, the perceived benefits of close collaboration between clinicians and scientists are not always realised and the gap between bench and bedside remains. Closer collaboration should expedite advances in treating disease. This important book, with a foreword by Fritz H Schröder, is designed to enhance the relationship between the two communities. It contains data from a multinational authorship, the thirty-eight chapters being written by experts with widely differing training and expectations. Their aim is to facilitate both understanding and in-depth knowledge of their subject. By bridging the gap between clinicians and scientists, the hope is to enhance translational research in their common goal of improving the diagnosis and treatment of prostate cancer, the single most important cancer affecting men./a

In recent years the pace of research in prostate cancer has increased dramatically. Creative ideas in combination with new and emerging technologies have led to an explosion of discovery. These types of advances in prostate cancer research presage an era of new treatment strategies based on an understanding of the cellular and molecular mechanisms of disease. In creating this book, we aimed to cover a broad "bench to bedside" research spectrum ranging from: genetic, molecular and cellular analyses to epidemiological studies, refinements in local treatment strategies and new biologically based non-hormonal treatments for systemic disease. Researchers and clinicians will find in this book a group of timely and clinically relevant chapters on prostate cancer research and treatment.

Prostate cancer is the most common noncutaneous prostate cancer. Research has revealed several distinct malignancy and the second leading cause of cancer mechanisms of castration-resistant disease that may deaths among men in the United States. It is a critical converge in patients with disease progression on public health problem and remains incurable in the ADT. Many approaches are currently being evaluated metastatic setting with mortality that usually occurs as to improve the treatment of this condition and these a result of castration-resistant disease. fndings have identifed several potential targets for Since Huggins and Hodges’ report of the dra- therapeutic intervention. These include drugs that are matic clinical effects of suppressing serum testos- more active or less toxic chemotherapy agents; drugs terone levels in men with advanced prostate cancer that induce androgen deprivation; drugs that target in 1941, hormone therapy (also called androgen the androgen receptor and/or androgen synthesis; deprivation therapy [ADT]) has become widely drugs that target specifc pathways, including ang- accepted as the mainstay of therapy for the treat- genesis and tyrosine kinase inhibitors, endothelin ment of advanced prostate cancer. ADT combined antagonists and matrix metalloproteinase inhibitors; with radiation therapy is a standard of care in the and immunologic approaches. Many of these agents treatment of men with locally advanced prostate seem promising and the rationale and effcacy of cancer on the basis of evidence that shows improved these emerging therapies remain to be validated in survival. The Clinically Organ Confined Adenocarcinoma Of The Prostate written by Hans-Peter Schmid and has been published by Springer Science & Business Media this book supported file pdf, txt, epub, kindle and other format this book has been release on 2012-12-06 with Medical categories.

Dr. Hans-Peter Schmid has addressed in this monograph the basic dilemmas surrounding prostate cancer. These dilemmas include the 40 % histologic incidence of invasive cancer in men in comparison to a 3 % death rate, the extraordinary slow doubling time of four to five years for untreated early clinical stages (T1 and T2 cancers), the limited life span of even healthy men over 70 years of age, and the national and international debates of how to treat this cancer. It is important, however, to recognize that despite these uncertainties, prostate cancer is probably the most predictive of all human solid tumors. Dr. Schmid has addressed the basis for much of this predictability in this monograph. The level of serum PSA and its rate of increase in the untreated patient with a prostate size of less than 50 grams is undoub tedly the most useful of all measurements in predicting volume and grade of the cancer, but it is closely followed by assessment of the extent and percent of grade 4 and 5 cancer in six systematic, spatially separated biopsies. Estimating the amount of suspected cancer on digital rectal examination by assigning a clinical stage is useful, but far less quantitative than PSA and systematic biopsies. The Basel series in this monograph adds strong support to these basic concepts. April 1994 ThomasA. Stamey, M. D.
