Excerpt from Cerebro-Spinal Fever This book has for its aim an attempt to bring together and correlate the clinical and pathological facts which we were enabled to accumulate during the epidemic of 1915. In January of that year, some of the first cases to occur in the Eastern Command were brought to the 1st Eastern General Hospital for treatment, and came under our care. The earlier cases were admitted in the first instance to different wards and were therefore under the charge of various physicians. Our thanks are due to Major Wright and Captains Curl and Haynes for allowing us every facility for studying the cases which had been admitted under their care. To Captain Curl we are especially indebted for much valuable counsel and help in dealing with these earlier cases. At the end of February the War Office appointed one of us bacteriologist to deal with the outbreak in the Western part of the Eastern Command. At the same time Colonel Griffiths arranged that a ward should be set apart for the treatment of all cases that arose. We were appointed to have charge of the cases admitted. A laboratory, which had been equipped by the Insurance Act Committee for purposes of research at the 1st Eastern General Hospital, was given for the investigation. In addition to providing the laboratory, the Committee assisted our investigations by appointing Mr H. W. C. Vines to study special problems as they arose. We wish to express our great indebtedness to the Insurance Act Committee for the equipment so generously given. To Mr Vines our thanks are specially due as much of his work has been incorporated in the present volume. Major Hele also rendered valuable assistance when the pressure of work was extremely great. In addition to laboratory work, it was the duty of the bacteriologist to visit the place of origin of every case, and investigate the hygienic conditions in which it arose. At this visit all contacts were examined to discover carriers. Ah proved carriers were at once brought into the special Cerebro-Spinal Fever ward, where they were kept under observation until two consecutive throat swabs had proved negative. Every case was therefore fully investigated by us from its commencement to the termination of the illness. Excerpt from Cerebro-Spinal Fever: The Etiology, Symptomatology, Diagnosis and Treatment of Epidemic Cerebro-Spinal Meningitis During the past few years a considerable amount of progress has been made in the diagnosis and treatment of cerebro-spinal fever. The outbreak of this disease in England in 1915 and the methods adopted to combat it afforded exceptional opportunities for its study. The authors of this volume were fortunately placed for the study and investigation of cerebro-spinal fever in that the one was a practising neurologist and the other a professional pathologist and bacteriologist. Being posted for duty at Woolwich they became responsible for the investigation, prevention and treatment of cerebro-spinal fever among the troops in the Woolwich military district. In the following pages are given the results of these investigations and observations extending over a period of three years, together with some observations upon cases which they had previously seen in civil life. In addition, the literature dealing with the disease has been consulted, and all work necessary to bring the subject thoroughly up to date has been incorporated. A full bibliography is appended. As the bulk of the work has been carried out while the authors were actively engaged in military duties, they are conscious that many imperfections may have crept into the text as a consequence. Nevertheless they hope that the book will prove useful to those who at any time may have to deal with an outbreak of cerebro-spinal fever either in an epidemic or sporadic form. The large number of troops at present mobilised in this country and America, as well as those that are returning from the various seats of war, render it an opportune time for the appearance of this book. Excerpt from A Treatise on the Continued Fevers The diseases considered in the following pages constitute a group with most of which the general practitioner is more or less familiar; it has, therefore, been my aim to describe them at greater fulness than is usual in the text-books, yet without the extreme elaboration that mars the usefulness of some of the special treatises. Brief historical sketches have been introduced, and considerable attention has been given to the subject of the special causes of particular diseases, as well as to their clinical phenomena and their anatomical lesions. Purely theoretical considerations have been, as a rule, omitted, and all controversial matters have been disregarded. The sections upon treatment are designed to represent recent practical knowledge, rather than to do justice to the changing opinions of which that knowledge is the outgrowth. With reference to the title of the book, it is to be admitted that, despite general usage and the highest modern authority, the classification of the infectious diseases, and in particular of those commonly known as the Fevers, is unscientific and provisional. Diseases being processes and not entities, are properly to be classified upon an etiological basis. In this case there were sickness at the stomach, vomiting, headache, great rest lessness, convulsions, and other symptoms, which, together with their speedy termination in death, warrant the opinion that she died of the prevailing epidemic. Later, when I told him that in my opinion lie had produced a valuable little book, he requested me to state the reasons for this favorable opinion in the form of a foreword. This I can do in a few brief statements. In the first place, on reading the manuscript, I soon became aware that the author had come to his task with not only a large experience behind him in the examination by various methods of the cerebrospinal fluid as an aid in diagnosis, but with a highly creditable record in the scientific study of this fluid as well. Evidently he had been drawn to his work on the cerebrospinal fluid because of its attractiveness as a field of research, as well as on account of its importance in diagnosis. It is to this happy combination of true philosophic interest and first-hand practical knowledge on the part of the author that the book owes its chief merit, namely, thoroughness and freshness in the parts dealing with fundamental problems, as well as in those dealing with practical matters. In the second place there could be no doubt in regard to the timeliness of a work of this kind. Indeed it seemed to me that a definite want would he supplied; for, in spite of an increasing importance in medicine, there was as yet no comprehensive book 011 all phases of the cerebrospinal fluid. With very few exceptions, I have been responsible for the treatment of all Of these cases. In any comparison of the cases in the present series with those Of past epidemics the great difference in age and sex incidence must always be kept in mind. All my cases occurred in adults, and with few exceptions in adult males who, a short time previously, had passed a medical examination as to their fitness for service with His Majesty's Forces. Excerpt from Report of a Committee of the Massachusetts Medical Society on Spotted Fever or Cerebrospinal Meningitis in the State of Massachusetts: May, 1866 The eruption which gave the name of Spotted Fever to the disease was not constant, and was less frequent in the year 1808 - 9, than in the two years preceding. Dr. Henry Fish, giving an account of the disease in Hartford in 1809, says that in that place it was without Spots, at least before death. The Committee of 1810 say it was not easy to de termine in how large a proportion of subjects the skin was affected with spots and eruptions. One of their observers had seen only four' instances of cutaneous affection in eighty cases, while another estimated it to have occurred in two thirds of all his cases. This last reporter, however, we are told, included very Slight affections. As to the character of the eruptions, the Committee say they were very various: Such as red Spots only, miliary erap tions, blotches, vesicles, sometimes resembling blood blis ters, pustules, and rarely purpura, petechias, or vibices. Sometimes there was an appearance like measles. Dr. Strong says the Spots did not rise above the surface, nor recede upon pressure.
