This research brief examines chronic kidney disease of unknown etiology (CKDu) in Sri Lanka. A new and growing public health challenge that has emerged in Sri Lanka, CKDu is epidemic in selected geographic areas of the country, primarily in its central and eastern dry zones. CKDu is not attributed to diabetes, hypertension, or other factors commonly associated with chronic kidney disease. A number of risk factors point to environmental triggers for the onset of CKDu, but evidence so far is insufficient to accurately pinpoint the potential cause or causes. Small studies conducted to date attempt to associate CKDu with agrochemicals, ayurvedic (traditional) medicines, water quality, or other environmental or lifestyle factors. This complex emerging health crisis requires an interdisciplinary approach, following strict field protocols and a rigorous environmental health risk assessment framework. Research and analysis to better understand CKDu are important for Sri Lanka and have global implications for understanding of similar geographic CKDu “hot spots” in Central America, Eastern Europe, and South Asia.

This manuscript undertakes a review of current published information (peer-reviewed and grey literature) on Chronic Kidney Disease of Unknown Etiology (CKDu) in Sri Lanka. It attempts to provide an overview of the possible environmentally-induced causal factors that have been implicated in the development of the disease, and identifies the gaps in research and recommends potential areas for future research. The review specifically captures the potential role that agriculture and water resources may play as causal factors in the development of the disease, and calls for a systematic approach and stresses the need for an integrated multi-disciplinary research effort to address the problem.

Noncommunicable diseases (NCDs) are the main cause of death worldwide. In addition, the incidence, prevalence, and death rates from NCDs are expected to increase in the future. No single discipline can address the issues of prevention, early detection, access to care, and appropriate treatment that are needed to improve outcomes for NCDs; this growing need will require transdisciplinary collaborations. Improving outcomes in NCDs is clearly a difficult endeavor that will require significant efforts. However, previous research and program implementation indicate that improving outcomes for NCDs is an achievable goal. Given the tremendous impact of NCDs on morbidity, mortality, and costs worldwide, there is little doubt that efforts to improve NDC outcomes are much needed. Recognizing the importance of new strategies and interventions addressing NCDS globally, RTI funded the projects described here to assess how the institute’s range of expertise could be focused on the reduction of NCDs. The projects described in this book focus on the risk factors or treatment (or both) of one or more of the four NCDs that are responsible for the greatest impact on morbidity and mortality: cardiovascular disease, cancer, diabetes, and chronic respiratory diseases. The pilot projects were carried out in low- and middle-income countries, which are disproportionately affected by NCDs. The lessons learned from these initial pilot projects have great potential to be increased in their scale and reduce the devastating impact of noncommunicable diseases.

The purpose of this Special Issue, “Water Intake, Body Water Regulation, and Health”, is to present novel reviews and experimental data regarding hydration physiology and its implication in overall health. Water has previously been dubbed the forgotten nutrient due to humans’ and animals’ ability to subsist seemingly unchanged across a wide range of daily water intakes. However, with the introduction of stressors such as exercise, diseased states, and/or chronic high or low water intake, the homeostatic signals related to body water regulation can influence organ and whole-body health. This Special Issue will discuss water intake, the scientific rationale surrounding the U.S. and European water intake guidelines, homeostatic mechanisms, diseases related to dysfunction of water regulation, and differences in the volume and the vehicle in which the water is contained (i.e., plain water versus mixed beverages) on water intake during and following exercise. The aim is to continue discussion surrounding water, the previously forgotten nutrient, and highlight the importance of water in daily life.

Residues offers readers a new approach for conceptualizing the environmental impacts of chemicals production, consumption, disposal, and regulation. Environmental protection regimes tend to be highly segmented according to place, media, substance, and effect; academic scholarship often reflects this same segmented approach. Yet, in chemical substances we encounter phenomena that are at once voluminous and miniscule, singular and ubiquitous, regulated yet unruly. Inspired by recent studies of materiality and infrastructures, we introduce “residual materialism” as a framework for attending to the socio-material properties of chemicals and their world-making powers. Tracking residues through time, space, and understanding helps us see how the past has been built into our present chemical environments and future-oriented regulatory systems, why contaminants seem to always evade control, and why the Anthropocene is as inextricably harnessed to the synthesis of carbon into new molecules as it is driven by carbon’s combustion.

Leptospirosis is the most common zoonotic disease worldwide, involving at least 1 million patients annually. Kidney is the preferential colonization site where Leptospira accumulates during systemic infection, primarily in the interstitium and in renal tubule epithelial cells. In acute kidney injury (AKI), the main histological findings are acute tubulo-interstitial nephritis and acute tubular necrosis, characterized by hypokalemia and non-oliguria. The injury caused by leptospira outer membrane protein components involves innate immunity Toll-like receptor pathways and activation of complement and immune cascades in the kidney, together with extracellular matrix accumulation when the kidney infection progress to chronic kidney disease (CKD). Significantly, recent studies indicate that leptospira may induce CKD in humans either with AKI-CKD continuum or subclinical leptospira infection leading to CKD. Leptospirosis kidney disease thus represents a newly discovered and previously overlooked area requiring intensive medical and public health attention and investigation. In this book, leading experts in the field contribute their cutting-edge clinical and research experiences for the understanding of leptospirosis kidney disease. The outstanding feature of this publication is emphasizing kidney injury and disease induced by leptospirosis, an underestimated infection involving multiple organs. The book contributes information on current knowledge of leptospirosis kidney disease and a new perspective for future research.
