This book is about the Diagnostic and Statistical Manual of Mental Disorders, more commonly known as the D.S.M. The D.S.M. is published by the American Psychiatric Association and aims to list and describe all mental disorders. Within its pages can be found diagnostic criteria for types of depression, types of schizophrenia, eating disorders, anxiety disorders, phobias, sleeping disorders, and so on. Also included are less familiar, and more controversial, conditions: Mathematics Disorder, Caffeine Intoxication, Nicotine Dependence, Nightmare Disorder. It must be admitted that the D.S.M. is not an exciting read. Its pages follow a standard format: Each disorder has a numerical code. This is followed by a description of the disorder, which includes information regarding prevalence, course, and differential diagnosis. Finally explicit criteria that patients must meet to receive the diagnosis are listed. These generally include lists of the symptoms that must be present, restrictions as to the length of time that the symptoms must have been troublesome, and clauses that state that the symptoms must not be better accounted for by some other condition.

Scholars question the extent to which current psychiatric classification systems are inadequate for diagnosis, treatment, and research of mental disorders and offer suggestions for improvement. In this volume, leading philosophers of psychiatry examine psychiatric classification systems, including the Diagnostic and Statistical Manual of Mental Disorders (DSM), asking whether current systems are sufficient for effective diagnosis, treatment, and research. Doing so, they take up the question of whether mental disorders are natural kinds, grounded in something in the outside world. Psychiatric categories based on natural kinds should group phenomena in such a way that they are subject to the same type of causal explanations and respond similarly to the same type of causal interventions. When these categories do not evince such groupings, there is reason to revise existing classifications. The contributors all question current psychiatric classifications systems and the assumptions on which they are based. They differ, however, as to why and to what extent the categories are inadequate and how to address the problem. Topics discussed include taxometric methods for identifying natural kinds, the error and bias inherent in DSM categories, and the complexities involved in classifying such specific mental disorders as “oppositional defiance disorder” and pathological gambling. This book provides a detailed examination of the questions that preoccupied British alienists throughout the second half of the nineteenth century. Was insanity one disorder with different forms or a set of distinct natural kinds that each had different causes, symptoms, and outlooks? Was it possible to devise a standardised classification of the insanities that provides a scientific basis to psychological diagnosis? Could statistics on psychological diagnosis provide data to help reveal the nature of insanity? The classification at the centre of these debates, the Medico-Psychological Association’s Table of the Forms of Insanity, caused deep divisions that took decades to resolve and hampered efforts to develop asylum medical statistics on psychological diagnosis. The use of the classification in national medical statistics was tantamount to being the standard classification for the asylum. As the appeal of statistics grew within medical circles, the debates intensified, and the divisions grew deeper. Despite lofty aims and years of debate, attempts to develop national statistics on psychological diagnosis had achieved very little by the beginning of the twentieth century. The failure of these efforts, hampered by the unwieldy processes adopted by Lunacy administration, led to the Table of the Forms falling into obscurity after its final set of revisions in 1932. In presenting for the first time the debates surrounding the Table of the Forms of Insanity, this volume calls for a re-evaluation of the history of psychiatric classification through its exploration of the underappreciated links between the standardisation of psychological diagnosis and the development of mental health statistics. By interrogating the links between asylum governance and the clinic, this book presents considerations on classification that still resound today, and provides valuable reading for scholars interested in the social history of medicine, the history of psychiatry, and the history of science.

This anthology of essays presents a sample of studies from recent philosophy of medicine addressing issues which attempt to answer very general (interdependent) questions: (a) what is a disease and what is health? (b) How do we (causally) explain diseases? (c) And how do we distinguish diseases, i.e. define classes of diseases and recognize that an instance X of disease belongs to a given class B? (d) How do we assess and choose cure/ therapy? The book is divided into three sections: classification, disease and evidence. In general, attention is focused on statistics in medicine and epidemiology, issues in psychiatry and connecting medicine with evolutionary biology and genetics. Many authors position the theories that they address within their historical contexts. The nature of health and disease will be addressed in several essays that also touch upon very general questions about the definition of medicine and its status. Several chapters scrutinize classification because of its centrality within philosophical problems raised by medicine and its core position in the philosophical questioning of psychiatry. Specificities of medical explanation have recently come under a new light, particularly because of the rise of statistical methods and several chapters investigate these methods in specific contexts such as epidemiology or meta-analysis of random testing. Taken together this collection addresses the question of how we gather, use and assess evidence for various medical theories. The rich assortment of disciplines featured also includes epidemiology, parasitology and public health, while technical aspects such as the application of game theory to medical research and the misuse of the DSM in forensic psychiatry are also given an airing.
