Purpose: The purpose of this study was to examine nurse's attitudes related to clinical alarm management. Methods: A cross-sectional descriptive study of nurses currently working in intensive care (ICU). ICU nurses were administered a Clinical Alarms Survey developed by the Healthcare Technology Foundation (HTF). Results: A total of 30 registered nurses who worked in an intensive care unit (ICU) setting responded to the survey. More than 83% of the respondents had greater than six years of experience. Strong evidence of an association between years of experience and nurse's perception about whether alarm sounds/visual displays should differentiate the priority of an alarm was established (p-value = .05). Overall attitudes noted that the majority of respondents (85%) agreed that differentiation of alarm sounds was important, and that nuisance alarms disrupt patient care. Participant responses were mixed (30% - Agreed vs. 50% - Disagree) on statements covering complexity of alarm system set-up and management and whether new technology had solved issues related to alarms. While 65% of respondents agree that the adoption of smart alarms would be reduce false alarms, only 10% of participants felt that current policies were being used effectively. Seventy percent of all participants ranked false alarms as most important concerning alarms and lack of training (57%) as least important. Conclusions The frequency of clinically insignificant, or technically `false' alarms continues desensitize nurses, leading to indifference to differentiate audible alarms. While technological advances in alarm design continues, prospective assessments of risks are needed help to identify priority of clinical alert importance, process failures, and needs potential for new equipment. Dissemination of new policy and revisions/updates to existing policy related to alarm management should occur regularly to increase awareness and compliance to established policy.

Reducing Alarm Fatigue in Critical Care Abstract This improvement project took place on the Critical Care Unit (CCU) of a non-profit hospital in Northern California. The unit houses 54 beds, employs over 210 employees, and houses the facility's central cardiac monitoring station which utilizes unit staff. The objective was to improve patient safety through reducing the risk of alarm fatigue by decreasing the total number of clinical alarms on the unit. Specified goals included a 20% reduction in the number of alarms sounding on the unit with a 20% reduction in telemetry utilization. Goals were chosen based on unit assessment findings in comparison to The Joint Commission's (TJC) National Patient Safety Goals and associated guidelines, as well as the American Heart Association's (AHA) guidelines for inpatient continuous cardiac monitoring. Stages of the project were implemented using Lewin's Change Theory. Findings include a 13% average decrease in telemetry utilization and a decrease in clinical alarms of 29%. The sustainability plan includes annual education modules regarding the alarm management policy and alarm event documentation, continued daily/ periodic auditing of the telemetry order process, and continued gathering of staff feedback through surveys and interviews.

Use of alarm management protocols compared to default alarm settings, of the monitored patients in the intensive care unit (ICU) result in fewer alarm-related patient injuries and deaths. Research evidence identifies the consequences of alarm fatigue on patient safety supports effective alarm management strategies and protocols however unit default settings and limits contribute to excessive alarms requiring no clinical action. Based on documented research results, the prevalence of heart monitoring alarms that do not require any clinical intervention has been estimated to range from 85% to 99% while default settings and limits are left unchanged. Implementing standardized alarm management protocols can minimize alarm sounds along with the consequences of alarm fatigue. Although individualized alarm management protocols are essential for each specific patient, the work flow including the environment within each specific unit are also important considerations toward enhancing alarm notification to efficiently facilitate alarm responses. If patient monitoring devices or alarms are not properly managed, it can have detrimental effects toward patient safety. Research findings indicate the reduction of alarms requiring no clinical intervention enhance caregivers response to alarms, resulting in increased patient safety.
