The important developments in cancer therapy in the past few years have been those regarding growth of systemic therapeutics. With enhancements in response rates in solid tumors, chances have appeared to improve the effectiveness of surgery. Administration of systemic therapy prior to surgery- neoadjuvant chemotherapy - presents one of the means by which clinicians have successfully shortened the extent of surgery and, in some cases, positively affected the clinical outcomes. The book collects works by expert clinicians from various disciplines of the field presenting a study of current advances in the function of neoadjuvant chemotherapy in different types of tumors.

This book is designed to present a comprehensive understanding for the rationale of neoadjuvant treatment sequencing for localized pancreatic cancer and to focus on accurate clinical staging and stage-specific treatment sequencing. Sections address important aspects of clinical management of localized pancreatic cancer from diagnosis to surgery. These areas include initial radiographic staging, management of obstructive jaundice, role of chemotherapy and chemoradiation in neoadjuvant therapy, assessment of treatment response, and operative considerations for complex vascular resections. A brief review of suggested readings addressing the particular topic follows in each section, as well as a summary of clinical “pearls”. Management of Localized Pancreatic Cancer: Current Treatment and Challenges provides a comprehensive resource which clearly defines the principles of neoadjuvant therapy as well as a clinical framework for successful therapy. It serves as a valuable guide to physicians of multiple disciplines who are interested in utilizing neoadjuvant therapy for localized pancreatic cancer.

In the chapters of this excellent text, the authors have described advances in the neoadjuvant chemotherapeutic management of a number of tumor types. Based on the extensive experience with this management concept in breast cancer, several chapters are devoted to this important topic (Section 1). Section 2 deals with the administration of neoadjuvant chemotherapy in non-breast malignancies. Those who read and study the content of the chapters in this book will come away with a far greater understanding of the rationale supporting neoadjuvant chemotherapy, the existing evidence-based data regarding its use, limitations of the approach, and required future investigative efforts to learn how to optimize the benefits of this innovative management concept.

This book aims to provide readers with a current overview of enrolling trials with immune-checkpoint inhibitors in the preoperative setting of localized bladder cancer, renal cell carcinoma, and prostate cancer. The advent of immunotherapy has revolutionized treatments of genitourinary malignancy and evolved strategies for multidisciplinary management. This book explains neoadjuvant checkpoint inhibitors in localized genitourinary cancers, providing insights into the mechanisms of response and development of resistance of cancer cells to immunotherapy. It debates optimal trial design of preoperative checkpoint inhibitors in GU tumors, including optimal endpoints and the role of pathologic response as a surrogate endpoint of survival. It also illuminates clinical management of patients with organ-confined GU tumors, such as side effect management and perioperative safety, before providing an overview of biomarker use for patient selection across the early-stage GU malignancies. Lastly, the book provides the reader with the most up-to-date data emerging from clinical trials involving immunotherapy in early-stage GU malignancies. Neoadjuvant Immunotherapy Treatment of Localized Genitourinary Cancers: Multidisciplinary Management provides a comprehensive review of the field, serving as a valuable resource for urologists, medical oncologists, pathologists, fellows in urologic oncology, upper-level residents training in urology and medical oncology, as well as pharmacists interested in RCC clinicians.

Despite recent advances in adjuvant therapies of cancer, the regi mens of postoperative adjuvant chemotherapy treatment which are presently available fail to cure the majority of cancer patients. Pre operative (neoadjuvant) chemotherapy represents a new approach in drug scheduling, based on sound theoretical, pharmacokinetic, and experimental principles. The preoperative timing of chemotherapy before definitive sur gery is not a minor change in the therapy of cancer. To be successful, large numbers of practitioners and their patients must participate. Substantial alterations of many aspects of the present management of cancer will have to follow. Therefore, before such therapy can be fully and routinely implemented, results of the novel treatment and its rationale have to be carefully evaluated. In preoperative treatment, other features will likely gain impor tance. For the first time, clinicians have a chance to follow the in vivo response of the tumor exposed to preoperative chemotherapy. The subsequent histological assessment of the tumor sample may likely become an important prognostic guide, permitting more re fined individual approaches to the planning of postoperative adju vant treatment. The value of such a treatment strategy can already be appreciated in the clinical setting, as seen from the therapy of osteosarcoma. Furthermore, preoperative chemotherapy might render previously inoperable tumors operable and hence resectable with a curative intention. The preoperative reduction of tumor bulk may also effectively decrease the need for more radical operations, permitting a more uniform adoption of conservative surgery.

Throughout the world, head and neck cancer is a major threat to public health and a significant challenge to both clinicians and basic scientists. Despite extensive efforts in primary prevention, screening, early detection, and therapy, long-term survival rates have not improved substantially in the last three decades. This book covers a wide range of exciting new findings in both clinical and basic sciences as they are relevant to head and neck cancer. These findings have recently enhanced our understanding of head and neck carcinogenesis at the genetic and molecular levels, offering the promise of improved preventive and therapeutic strategies. This book will also present information on the important clinical advances that have been made in chemoprevention, organ preservation, and the simultaneous use of chemo therapy and radiotherapy. The first part provides an overview of the etiology and biology of head and neck cancer, including an examination of human papillomaviruses in both benign and malignant lesions. This section also discusses the carcinogenic process at the genetic and molecular levels, as well as aberrant squamous differentiation; increased understanding of these areas has great potential to translate into new strategies for cancer prevention. The second part describes recent advances in developing a risk model for head and neck cancer, as well as the application of genetic susceptibility data in chemoprevention. This section also includes overviews of the status of chemoprevention trials and of the process of invasion and metastasis in head and neck cancer.
