Even though clozapine is the only drug with evidence-based efficacy in schizophrenia symptoms not responding to other agents, it is still under-prescribed in most countries around the world. The barriers for initiating clozapine are evident in both clinicians and patients. Prescribers mainly report delaying clozapine because of fear of severe side effects and lack of knowledge regarding management of side effects driven by stigmatic rather evidence-based views. Patients report reservations toward the need for safety regulation and blood tests. Enduring psychotic illness is often associated with high level of burden of symptoms, which does not allow progress into recovery process in the community. It is often the case that treatment with clozapine has a dramatic effect on symptoms, otherwise refractory. However, there cases in which there is only minor or no effect of the drug. In these cases, sometimes referred to as ultra-resistant schizophrenia, it is advised to optimise clozapine treatment using pharmacokinetic considerations and confirm sufficient clozapine serum levels using therapeutic drug monitoring. Further steps include precise diagnosis and management of other physical or psychiatric comorbidities, offering targeted psychotherapy such as CBT and family interventions and eventually to wait, as sometimes there are cases of late response. In this book we aimed at providing clinicians with practical, up-do-date knowledge to support their decision-making and safety management when prescribing clozapine. It is our hope that using the information in this book would encourage both prescribers and patients to consider clozapine treatment when it is needed, in a timely fashion. We believe that accurate and systematic information is a key for reducing anxiety and relieving stigma associated with clozapine treatment.

Clozapine is an antipsychotic agent that has proved to be effective in treatment-resistant schizophrenia. Its use is reserved for patients unresponsive to other medication or intolerant of alternatives due to their adverse effects . It may also be prescribed for psychosis associated with Parkinson’s disease where conventional treatment has failed. In treatment-resistant schizophrenia, clozapine should only be prescribed by physicians well versed in the use of clozapine therapy in psychiatric illness or by psychiatrists. In psychosis associated with Parkinson's disease, clozapine may also be prescribed by a neurologist. When treated with clozapine many schizophrenic patients can be managed in the community. Introduction: Clozapine represents an irreplaceable therapeutic strategy, particularly in treatment-resistant schizophrenia (TRS) therapy, which, however, carries certain risk.Objects: The aim of the study was to evaluate frequency of clozapine adverse effects in patients with schizophrenia-spectrum disorders hospitalized at Department of Psychiatry, University Hospital Brno, during the period 2012-2014.Methods: Data were evaluated retrospectively, out of total number of 336 patients (285 patients with F200, F231, F232, and 51 patients with F25), 62 patients (41 men and 21 women) were treated by clozapine. Results: Clozapine doses ranged from 100-700 mg/day in men, 150-600 mg/day in women, mean dose was approx. 300 mg/day. Adverse effects of clozapine included tachycardia (50 %), constipation (34 %), increased weight (15 %), tiredness and blood dyscrasia, manifested as neutropenia in four patients, none presented agranulocytosis. With lower frequencies were noted hypertension, salivation, insomnia.Conclusion: Although a relatively wide spectrum of adverse events has been reported with high frequency, none of the adverse events led to clozapine discontinuation or was classified as severe. The benefits of clozapine therapy undoubtedly outweigh the risks which can be reduced by close monitoring and adequate interventions combined with therapeutic strategies u2013 e.g. betablockers for tachycardia, dietary interventions, laxatives for constipation and dietary measures for metabolic side effects together with pharmacotherapy of dyslipidemia, pre T diabetes/diabetes or obesity. The most serious risk of clozapine therapy is agranulocytosis, occurring with prevalence of 1-2 %, regular blood screening as recommended and early therapy can prevent cases of this life-threatening condition and eliminate the risk.
