Childhood epilepsy may cause cognitive disorders and the intellectual quotient is indeed not normally distributed in epileptic children, a fair proportion of whom show an IQ in the deficient range. Some epileptic syndromes happen during vulnerability periods of brain maturation and interfere with the development of specific cognitive functions. This is the case for the Landau–Kleffner syndrome, which generally appears during speech development and affects language. Similarly, West syndrome – or infantile spasms – is an epileptogenic encephalopathy appearing during the first years of life and induces a major delay in social and oculo-motor development. Specific impairments can also be identified in partial childhood epilepsies in relation with seizure focus localization. For instance, left temporal and frontal epilepsies are frequently associated with verbal impairments. Moreover, episodic memory disorders have been described in children suffering from temporal lobe epilepsy whereas executive deficits (planning, self-control, problem solving) have been reported in frontal lobe epilepsy. In most cases, including its mildest forms, childhood epilepsy induces attention deficits, which may affect academic achievement. These observations militate in favor of individual neuropsychological assessments as well as early interventions in order to provide the child with an optimal individualized treatment program.

This book is devoted to the neuropsychological description of childhood epilepsy, a neurolo- cal condition that constitutes one of the most prevalent forms of chronic and disabling childhood illnesses. Indeed, one child out of 20 experiences one or more seizures before the age of 5, and one in a hundred develops epilepsy as a chronic disorder. Approximately half of these children with epilepsy display academic difficulties and/or behavioral disorders. Moreoever, it is now believed that a sizable proportion of children with learning disability suffer from undiagnosed epilepsy. While a great number of textbooks have been devoted to various medical aspects of chi- hood epilepsy (diagnosis, genetics, etiology, drug and surgical treatment, etc.), there have been no comprehensive accounts of the cognitive consequences of this condition. Advance of medical knowledge has shown that childhood epilepsy should not be considered as a single disorder but encompasses a whole range of different conditions that exhibit specific clinical EEG and outcome characteristics. It is not becoming apparent that these various clinical entities have different cognitive expression that yet need to be specified. To date, little research has been conducted to the effects of epilepsy on cognitive and language issues. This study is a response to this knowledge gap. A careful matched-subject, mixed-factors description, designed to identify cognitive decline during the process and treatment of this illness. The research is focused on description and exploration, of linguistic considerations in Rolandic epilepsy. Rolandic epilepsy, is also known as Benign focal epilepsy with centrotemporal spikes (BECS). The intention is to shed light from a neurolinguistic perspective, on reasons that might underline cognitive and behavioral complications, in children with epilepsy. This study has been approached corroborating ‘structural connectivity’, which defines language network in the brain. The rational derives from research of neurological data, from prestigious neurological and scientific international journals, on the topic that illustrates our case. Conclusions are based upon careful evaluation on how epilepsy affects cognitive skills, and language issues upon observations drawn on results. Deficits in language patterns have been associated to different brain areas, in patients who suffer from different pathologies. Degenerative illnesses have opened a milestone in the study on reasons leading to cognitive decline, in children affected from “Benign childhood epilepsy”. Medical history has been well documented to current day. Lack of consistent information internationally recognized on cognitive issues toughen consensus on best medical procedure to follow, in best interest of the child. Our research confronted this shortage of information, in the light of the new literature coming through. Overall, the results indicate that when working with language skills in people with epilepsy or with degenerative diseases, clinicians should account for the presence of anti-epileptic medication or amalgamation of medications as possible cause of cognitive decline.

What are the consequences, if any, of repeated brief seizures that are the defining feature of epilepsy? A firm answer to this question has been surprisingly elusive for a variety of reasons. Clearly there is a subset of patients who appear to tolerate seizures with relatively limited long-term consequences, and not all patients are destined to progress to intractability with frequent seizures and disability. This variability and individual susceptibility has made it difficult to make statements that fairly apply to the full range of people with epileptic disorders, whose disorders span a broad spectrum from mild with excellent control and few limitations, to severe with multiple daily seizures and pronounced disability that affects employment, educational performance, an personal life. This volume seeks to explore the spectrum of severe to more subtle damage that may be a consequence of seizures. Having appeared in the 1930s in Montreal, standardised neuropsychological evaluation has become an essential tool in the clinical diagnosis and evaluation of surgical epileptic patients. Epilepsy surgery is now widely accepted as an effective therapeutic option for carefully selected children with medically refractory epilepsy. The surgical procedure may cause cognitive deficits or exacerbate existing impairments, but it may also improve cognitive abilities by the restoration of functions located in adjacent or contralateral areas that had been secondarily affected by the epilepsy or the underlying pathology. Compared to adults, better cognitive outcome has been reported in children, a finding probably due to the developing state of the brain, which possesses considerable structural and functional plasticity. More extensive and effective surgery such as hemispherectomy is more commonly used in the pediatric population, and this must also influence surgical outcome. However, studies related to cognitive outcome of epilepsy surgery in children are limited, and controversial results are often reported. In this chapter, we provide a current overview of the literature on cognitive outcomes in children who undergo different types of epilepsy surgery, including focal resections as well as corpus callosotomy and hemispherectomy. Early surgical intervention appears to be a rational option for the treatment of childhood epilepsy since many cognitive deficits are linked to the epileptic process and may disappear when seizures are controlled.
