The publication of this volune represents the first in a planned series of special publications on topics of major interest to workers in the fields of psychiatry and psychology. The series will be en titled The Downstate Series of Research in Psychiatry and Psychology and is the outcome of a series of discussions among several senior members of the Department of Psychiatry held about four years ago. Included in this group, were Drs. Benjamin Kissin, Henri Begleiter, Leonard Rosenblum, Herbert Pardes, Norbert Freedman, and myself. The talks were initiated by Dr. Pardes, now Chairman of the Department of Psychiatry at the University of Colorado, and the resultant decision was t6 hold three day symposia, hopefully of such excellence that the published papers of each symposium would represent a significant con tribution to some particular aspect of human psychology. This deci sion necessitated the choice of suitable topics and distinguished speakers who would present original work coordinated into an inte grated framework based upon a selected topic.

The psychological states of patients with diabetes mellitus were compared with those of patients suffering from other chronic diseases and people with no diagnosed chronic diseases. These states were assessed by applying content analysis scales to transcripts of their descriptions of their current experiences. Analyses of the diabetics' scale scores re vealed a pattern characterized by much anxiety, depression, anger expressed both direct ly and indirectly, together with feelings of helplessness. The sources of anxiety which proved to be of most importance to them were fears of death and bodily mutilation, as weIl as guilt and shame. They experienced little sense of sharing with most people around them, although they showed considerable enjoyment of dose relationships with family and friends. This pattern of psychological states did not vary with the sex of the patients or whether they were interviewed in a hospital or at horne nor with recency of onset or multiplicity of health problems. It was similar to the pattern of patients with other chronic diseases but differed significantly from that of the healthy group. Although clinical interpretation originated with Freud, the latter's positivist preference for purely observational methods made him ambivalent toward interpretive methods. According to Rubovits-Seitz, the legacy of Freud's positivism still pervades clinical thinking and interferes with progress in investigating and improving interpretive methods. He reviews the paradigm shift in general science from positivism to postpositivism by way of demonstrating the compatibility of interpretive inquiry with a postpositivist approach. Post-Freudian models of clinical interpretation are evaluated, andclinical methods of interpretation are compared with interpretive approachesin nonclinical fields. A detailed discussion of the neglected problem ofjustifying interpretations incorporates evaluations of specific justifyingprocedures and a case report illustrating applications of such methods.
